Applications for registration of medical institutions under Article 25 of the Rules for the Implementation of the Regulations Governing Medical Institutions must fill out the "Application for Registration of Medical Institutions" and submit the following materials to the registration authority:
(a) "approval letter for the establishment of a medical institution" or "set up a medical institution for the record";
(b) proof of property rights of the premises of a medical institution or proof of use;
(c) proof of ownership of medical institutions or proof of use;
Applications for registration of medical institutions must be submitted to the registration authority. Proof;
(3) medical institution building design plan;
(4) proof of capital verification, asset evaluation report;
(5) medical institution rules and regulations;
(6) medical institutions, legal representatives or principals, and the directory of heads of departments and relevant qualifications, practice certificates and copies of certificates;
(7) province, Autonomous regions and municipalities directly under the Central Health Planning Administration to provide other materials.
Applications for registration of outpatient clinics, clinics, health centers, medical clinics, health care centers and health stations should also submit a list of the types of medicines attached to the pharmacy (cabinets), a directory of health professionals and their qualifications and certificates of practice, as well as other materials prescribed by the provincial, autonomous regions, municipalities directly under the central health administrative department to submit.
