I. General requirements
The physical examination of the 20 19 college entrance examination in our province was carried out in strict accordance with the Notice of the Ministry of Education, the Ministry of Health and the China Disabled Persons' Federation on Printing and Distributing the Guidance Opinions on the Physical Examination of College Entrance Examination (J [2003] No.3). Second, the physical examination unit
20 19 The physical examination of the college entrance examination shall be carried out by the education, health and health departments at or above the county level (hereinafter referred to as the "Health and Health Commission") and the recruitment organization. The physical examination of the college entrance examination must be carried out in a designated physical examination hospital or the corresponding medical unit approved and announced by the Provincial Health and Wellness Commission (see Annex 1). The chief physician should be a doctor with the title of deputy chief physician (inclusive) or above and a strong sense of responsibility. The designated hospitals for college entrance examination shall be determined by the education, health and health departments of counties (districts) in consultation with the recruitment office and announced to the public. It is strictly forbidden to take the college entrance examination in hospitals below Grade II and Grade A. ..
Third, the physical examination object
Candidates who sign up for the unified entrance examination of colleges and universities in Jiangxi Province on 20 19 and enroll students separately in advance (including counterpart enrollment) must take the college entrance examination. Candidates who fail the college entrance examination are not allowed to be admitted to colleges and universities.
Fourth, physical examination items
From 2065438 to 2009, the physical examination items of general college entrance examination in our province were determined according to relevant regulations. Please refer to the sample physical examination form for detailed physical examination items (see Annex 2).

Before the college entrance examination, cities, counties (districts) and middle schools should take the initiative to remind candidates to fill in their past medical history (including whether they are cured or not, current corresponding indicators, etc.). ) detailed and accurate, and be responsible for the authenticity. If they have no previous medical history, they must fill in "None". If it is found that they deliberately conceal their past medical history after entering school, the school has the right to handle it according to the school's enrollment regulations. Remind medical examiners to be responsible for candidates and enrollment institutions, conduct a serious college entrance examination for candidates, make corresponding standardized and accurate conclusions on candidates' physical examination according to Document No.3 [2003] of the Ministry of Education and other ministries and commissions, and carefully fill in the relevant columns on the physical examination form as required. The conclusions of all inspection items are filled in by medical examiners with black signature pens.
(a) vision, blood pressure, height, weight, hearing, liver and spleen size and other physical examination items need to use a black pen to fill in the specific values in the corresponding boxes. Vision measurement adopts the international standard visual acuity chart, and the blood pressure measurement unit stipulates the unified use of mmHg (that is, mmhg).
(2) For other inspection items except those mentioned in (1), the code of the result option shall be filled in the corresponding box or recorded in words; If the option is other, you must fill in the specific situation in words in other columns of each subject.
(3) Doctors who take part in the college entrance examination should seriously study the Guiding Opinions on Physical Examination of Enrollment in Colleges and Universities, and give the physical examination opinions of all subjects in the opinion column of physical examination doctors in all subjects (fill in the number: 1 qualified, 2 professional restrictions, 3 unqualified; The diseases listed in the first part of the Guiding Opinions on Physical Examination of Enrollment in Colleges and Universities are unqualified contents of physical examination, and the second part is professional restricted contents); And sign the full name in the column of "inspector" and "doctor's opinion" (if both belong to the same person, just sign the full name in the column of doctor's opinion) to show responsibility.
(4) Each physical examination unit shall designate a special person to give the candidates' physical examination conclusion in the column of "Opinions of Physical Examination Hospital or Station" according to the opinions of doctors in various departments and the results of physical examination. In the four option boxes of "Qualified", "Unqualified", "Limited Specialty" and "Unsuitable Specialty" and the specific corresponding item of "Guidance Opinion", the corresponding information boxes are blacked out with a black signature pen, and the official seal or special seal of the blue ink unit of the medical examination hospital (medical examination station) is stamped in the specified area to avoid pollution to the information box of the medical examination conclusion.
Sixth, the schedule.
(a) physical examination in principle should be completed before April 20. The specific time shall be arranged by the recruitment offices of cities and counties (districts) in consultation with the relevant medical examination hospitals. Each district and city recruitment office shall summarize the time schedule of college entrance examination in each county (district) under its jurisdiction and the name of the designated medical examination hospital, and submit it to the Provincial Education Examinations Institute before March 3/KLOC-0.
(2) All districts and cities will send the physical examination forms of all candidates in the region to the provincial education examination institute for scanning and archiving according to the relevant information management regulations before May 3.
(3) Candidates can log on to the website of Jiangxi Education Examinations Institute (www.jxeea.cn) after May 15 to check the physical examination results. Counties (districts) should print the conclusion of physical examination and inform the candidates. Candidates who have doubts about the conclusion of physical examination can apply for a second interview to the county (district) admissions office, which will be submitted to the Provincial Education Examinations Institute for a second interview after being summarized by the districts and cities. The deadline for reporting is May 24th. Candidates who apply for re-examination or final exam can check the physical examination results after June 10.
Seven. Re-inspection and final inspection
(1) Apply for re-inspection and final inspection. City, county (District) Admissions Office and relevant middle schools have the obligation to explain and guide the candidates in doubt when reading the results of physical examination. Candidates who have objections to the conclusion of the physical examination may apply for a re-examination to the city or county (district) where they are registered according to the prescribed time and procedures, and the re-examination work shall be carried out by the physical examination hospital designated by the local recruitment office. The re-examination fee shall be borne by the candidates themselves.
Candidates who still have objections to the re-examination results must apply for a re-examination to the local recruitment office before May 20. Candidates who fail to take the physical examination or re-examination according to the requirements of the local recruitment office may not directly apply for the final examination. Height, weight, vision, color vision, blood pressure, etc. are not checked in the check-up hospital. Candidates who have objections to these five physical examination conclusions apply for re-examination according to the re-examination application process, and the re-examination conclusions will be the final physical examination results of the corresponding projects.
(2) finally check the hospital. Jiangxi Provincial College Entrance Examination Committee and Provincial Health and Wellness Committee designated Jiangxi Provincial People's Hospital as the final physical examination hospital for the general college entrance examination in our province, and was responsible for making a final ruling on the disputed physical examination conclusion. Jiangxi Provincial People's Hospital should strengthen its leadership in this work and implement the responsible departments and persons. Candidates who pass the final inspection hold their ID cards and go to Jiangxi Provincial People's Hospital for the final inspection according to the specified time. The final examination fee shall be borne by the candidates.
(3) Determination and treatment of abnormal transaminase. According to the document [2010] No.22 of the Teaching Department of the College Students Department of the Ministry of Education, the serum alanine aminotransferase exceeds the upper limit of the reference value by more than 2 times (including 2 times), and it should be re-examined by B-ultrasound. All cities and counties (districts) recruit candidates who remind that the transaminase exceeds the upper limit of the reference value (the standard is 40IU/L) for more than 2 times (including 2 times) to have blood tests again in the same local college entrance examination hospital. Candidates who have objections to the conclusion of the re-examination need to sign up for the re-examination according to the re-examination application procedures of the city and county (district) recruitment office, and the re-examination project is B-ultrasound. If the reexamination results are "negative", "no abnormality" and "fatty liver", the physical examination conclusion shall be deemed as qualified; Other conditions shall be truthfully recorded by the re-inspection hospital, and the physical examination form shall be filled in. Candidates who still have objections to the re-examination results can apply according to the re-examination procedures stipulated by Jiangxi Education Examinations Institute, and the re-examination project is liver CT examination. Candidates whose transaminase exceeds the upper limit of the reference value (standard 40IU/L) by more than 2 times (including 2 times), but who have not participated in the re-examination, the admissions institution may not accept them. Candidates who take part in the re-examination or final inspection, if the inspection conclusion does not belong to the above-mentioned circumstances that can be considered as qualified, Jiangxi Education Examinations Institute will truthfully provide inspection records to the relevant enrollment institutions during the admission process, and the enrollment institutions will decide whether to admit them.
(4) Protecting the rights and interests of candidates. Candidates who have objections to the conclusion of the physical examination shall be arranged by the city and county (district) recruitment office for re-examination and final examination opportunities. Candidates who apply for the re-examination and final examination shall be subject to the results of the re-examination and final examination of the corresponding items in their electronic files. For items that are not re-examined according to the above-mentioned prescribed procedures, the conclusion on the physical examination form shall be taken as the final physical examination result, and the conclusion drawn by candidates who go to the hospital by themselves shall not be accepted.

Education, health and health departments at all levels and recruitment offices should earnestly strengthen the leadership of physical examination, attach great importance to it, organize it carefully, cooperate closely, implement measures and strictly manage it. It is necessary to establish and improve the institutions, rules and regulations and supervision mechanism of the college entrance examination, and earnestly do all the work of the 20 19 college entrance examination.
Hospitals designated for the college entrance examination have the responsibility and obligation to do a good job in the college entrance examination as required, and may not refuse to accept the college entrance examination for various reasons without special circumstances. The province's college entrance examination fee shall not be higher than 60 yuan/person (including initial examination, physical examination scanning, data entry, etc.). ) stipulated by the provincial development and Reform Commission and the provincial department of finance.
(B) to strengthen business training
Municipalities, counties (districts) education, health departments, physical examination hospitals and enrollment offices should strengthen the pre-job business training for all staff participating in the college entrance examination, and those who have not been trained or failed in the training may not take up their posts. All physical examination hospitals must strictly follow the guidelines of Jiao [2003] No.3, Ministry of Human Resources and Social Security [20 10] 12, Teaching Hall [2010] No.2, Education Letter [2010] No.22 and the Ministry of Education on how to do a good job in 20/KLOC. The seal is required to be clear, and graffiti and alteration are strictly prohibited.
(3) Standardize physical examination management.
1. Carefully verify the identity of candidates. The class teacher is responsible for the on-site inspection of middle school students, strictly checking the candidates themselves, ID cards and the roster of students in this class; Social candidates by the city, county (District) recruitment office staff responsible for on-site verification, strictly check the candidates themselves, identity cards, online registration form photos and other information, to prevent the occurrence of impersonation.
2. Strengthen the management of medical examination places. All medical examination hospitals should ensure the order of medical examination places and arrange special personnel to be responsible for guiding candidates. The medical examination place should be separated from the outpatient clinic, and parents or relatives of candidates are prohibited from entering the medical examination place. Medical personnel should wear work permits. During the medical examination, the city and county (district) admissions office should send people to coordinate related matters on the spot to help maintain the medical examination order. City, county (District) recruitment office and relevant medical hospitals should formulate emergency plans to prevent accidents.
3. Do a good job in organizing physical examination. Each middle school (registration point) should assist the city and county (district) recruitment office and the health department to organize candidates to participate in the physical examination. Candidates must truthfully fill in the past medical history and disability, and must not be blank. The physical examination hospital should have a special person to guide the candidates to participate in the physical examination, and be responsible for the handover and custody of the physical examination form. During the whole medical examination, candidates are not allowed to touch the medical examination form. During the physical examination, the physical examination physician shall inform the candidates of their eyesight, correction degree and blood pressure on the spot. Candidates with questions will be re-examined on the spot. After the physical examination, the above items will not be re-examined.
4. Keep abreast of the physical examination. City, county (District) recruitment office should keep abreast of the physical examination process, understand the special problems found in the physical examination, make records, report them in time and properly handle them.
5. Strengthen the supervision of the medical examination process. During the physical examination, cities and counties (districts) should actively coordinate the participation of discipline inspection and supervision personnel in supervision. If it is found that the physical examination site is in disorder, illegal operation, fraud, impersonation and other disciplinary actions, it must be reported in time and assist the education and health departments to investigate and deal with them.
6. Strengthen the awareness of serving candidates. Education, health care departments and recruitment offices of cities and counties (districts) should enhance their service awareness, establish the idea of people-oriented and serving candidates, actively cooperate with medical examination hospitals to arrange medical examination links reasonably according to the number of local candidates, and avoid candidates waiting for a long time, especially for disabled candidates and candidates with physical defects. We should patiently accept candidates' consultation, carefully explain the medical examination restrictions and majors that are not suitable for study, and do a good job in consulting services.
7. Strict accountability for violations of discipline. Physical examination of college entrance examination is a policy and technical work. Education, health and health departments at all levels and recruitment offices should adhere to the principle, strictly control, be accurate, and ensure the smooth progress of medical examination. The principle of "who checks, who signs and who is responsible" is implemented in the physical examination of college entrance examination. Physical examination personnel should strictly abide by work discipline. If fraud, malpractice, dereliction of duty and mistakes are found and verified in the college entrance examination physical examination, they will be dealt with seriously according to the law and regulations.
