New Agricultural Cooperative Reimbursement Procedure:
Required Information:
1. Outpatient Reimbursement Carrying Information: Outpatient Invoice, Cooperative Medical Card Calendar (or medical records).
2. Hospitalization reimbursement: hospitalization invoice, cooperative medical certificate (or medical record), detailed cost list, discharge summary, and other relevant certificates.
3. Outpatient reimbursement for special diseases: invoice for outpatient treatment, and medical certificate for special diseases.
4. For special diseases. Documents to be carried: outpatient treatment proposal for special diseases, cooperative medical certificate book, medical records, relevant laboratory reports, and two photos.
Reimbursement process:
1. Participating households will be reimbursed for the information required to be submitted to the village (community) cooperative medical liaison officer by the village (community) cooperative medical liaison officer after review and report to the town cooperative reimbursement. 2. Hospital direct reimbursement: when hospitalized for hospitalization due to illness, the new rural cooperative medical card issued to the hospital directly involved in the reimbursement.
