(1) Medical conditions:
1. The patient must have histologically or cytologically confirmed primary stage IIIB or IV non-small cell lung cancer.
2. The patient knows that he/she has advanced lung cancer and has been evaluated by a designated medical institution and confirmed to be eligible for Trocar? Patients who have been evaluated by a designated healthcare provider and confirmed to be eligible for the indication of Trocar?
3. Patients must have an evaluable lesion prior to treatment with Terokai? treatment must have an evaluable lesion prior to treatment.
4. Prior to application, patients must be continuously taking Roche Trocar? (150 mg/tablet/day) therapy for at least five months until the time of application, with no disease progression and clinically assessed to be able to continue to benefit from Trocar? treatment. (This item can be ignored for patients with low income)
5. Patients will be medically followed up according to the time specified in the program process and provide appropriate examination reports, and the hospital implementing the program will provide an assessment report to confirm that there is no disease progression and that the patient needs to continue to take Terokai? (150mg/tablet/day) treatment without intolerable toxic side effects.
(2) Other conditions:
1. The assistance of this program is provided to mainland patients holding Chinese People's **** and National Resident Identity Card/Military Officer's Card.
2. A low-income certificate issued by the personnel department of the organization and the people's government of the street/township where the household registration is located, issued by the person himself/herself in accordance with his/her working status, which meets the following requirements.
- For those who do not have a work unit, it must be signed and stamped by the person in charge of the local government at or above the township level or the street office.
- For those who have a work unit, it must be signed and stamped by the person in charge of the personnel department of their unit and the person in charge of the local government at or above the township level or the street office.
3. Low-income patients are required to provide a copy of the certificate of low-income insurance issued for more than one year with the official seal of the issuing authority, a copy of the record of receiving the minimum subsistence allowance in the previous year (with the seal of the receiving unit) and the original copy of the certificate of low-income insurance of the Terokai? Charitable Gift of Drugs Program original certificate of low income insurance.
4. Patients are required to fill in the program reservation form within two months after starting to take Terokai? Within two months after starting to take Terokai, patients should fill in the program reservation information form and informed consent form and send them to P.O. Box 100034-88, Beijing by EMS or registered mail to make a reservation for the program. Patients with low-income conditions can apply directly.
Termination conditions
The grant will be automatically stopped if one of the following conditions exists
1. Disease progression is confirmed by the hospital where the program is implemented
2. There are intolerable toxic side effects
3. The patient or legal guardian/relative requests to stop the continuation of the Trocar? treatment
4. Failure to insist on regular follow-up visits by the executive physician at the charity designated hospitals
5. Patients who refuse to undergo the program review
6. Failure to provide medical information to prove that their tumors have not progressed as scheduled
7. Death of the patient who receives the assisted treatment
8. Provision of any inaccurate or false medical or financial certificates
9. Sale or transfer of the complimentary drug to another person
10. Patient on Trocar? May be eligible for reimbursement of expenses
11. Force majeure or for special reasons must stop the complimentary drug program
