What evaluation materials should be provided by the employer who accepts the evaluation and audit of occupational disease prevention and control?

Preparation of evaluation materials

(a) the employer who accepts the centralized material review shall provide the following assessment materials under the organization of the municipal (prefecture) level safety supervision bureau:

1. List of main persons in charge of the employing unit, occupational health training (or training with occupational health content) assessment certificate issued by the safety supervision department or its entrusted institution, or other certification materials;

2.20 13 occupational health training records or other supporting materials of the employees of the employer before taking up their posts;

3. The employer's occupational disease hazard project declaration form;

4. The summary report of the occupational health examination report of the employing unit in the past two years, and the original or photocopy of the occupational health examination form for employees of not less than 10;

5.20 13 test report on occupational hazard factors in the workplace of the employer;

6. The original or photocopy of the labor contract signed between the employer and the employee in the last two years (if the number of employees in the unit is large, the original or photocopy of not less than 10 shall be provided);

7. An overview of the setting of occupational hazard warning signs by employers in different workplaces;

8.20 13 Records of the Employer's Acceptance of Supervision and Inspection of Occupational Hazards Prevention and Control in the Workplace.

(two) the employer who randomly accepts the spot check shall provide the following assessment materials on the spot:

1. Staff roster, number of people exposed to occupational hazards and their post distribution;

2. List of main persons in charge, occupational health training (or training with occupational health contents) issued by the safety supervision department or its entrusted institution, or other certification materials;

3.20 13 annual employee occupational health training records or other supporting materials before taking up the post;

4. Occupational hazard project declaration form;

5.20 13 annual occupational disease hazard detection report;

6. Occupational health examination report in recent 2 years;

7. The labor contract signed by the employer and the employee;

8.20 13 accept the supervision and inspection records of occupational hazards prevention and control in the workplace.
