2. What documents do I need to submit to apply for a health code?
(1) Pre-trip test report (please take samples after vaccination 14 days, and the booster is not limited to 14 days).

2. If the non-inactivated vaccine is completely inoculated, please submit the negative report of 1 nucleic acid test and the negative report of serum (IgM) anti-N protein antibody test;

(2) Vaccination certificate
Whole-course vaccination of COVID-19 vaccine refers to those who have completed the whole-course vaccination of inactivated or inactivated COVID-19 vaccine (the dosage and interval meet the requirements) 14 days. At present, the common inactivated vaccines in COVID-19 are Sinopharm (2 doses) and Kexing (2 doses), while the non-inactivated vaccines in COVID-19 are Pfizer (2 doses), Madner (2 doses) and AstraZeneca (2 doses).
1. For those who have been completely vaccinated in China, please apply for the People's Republic of China (PRC) International Travel Health Certificate (the sample table is as follows) after handling the international epidemic prevention health code; In case of special circumstances, if it is impossible to obtain a unified voucher by inquiring vaccination records through procedures, please submit the official vaccination voucher issued by the official authority of China and the vaccination statement of COVID-19 at the same time (see Annex 1, and China citizens must write their full names in block letters).
2. Those who are fully vaccinated in New Zealand should apply for the international vaccine certificate "International? Travel? Vaccinate? Please submit an international flight ticket or e-ticket issued by an airline or travel agency with its origin in New Zealand and its destination in China, which must include the applicant's name, flight number, departure date and other basic information. Booking orders without personal and flight information and screen shots of mobile phone booking messages are not accepted.
(5) Pre-trip self-health management certificate
Please start self-health protection on the 8th day before departure, and truthfully fill in the closed-loop management commitment letter of self-health monitoring and isolation (Annex 5). The rapid antigen self-test (RATs) must be started after the negative results are confirmed, and the personal temperature should be measured on time and recorded truthfully every day from the 8th day before boarding, until the day before departure (for example, passengers on the 14 flight on March 7th should start and end on March 3rd).
There is no need to upload the electronic version of the application procedure. Please print and sign it (in block letters, the baby is signed by the peer guardian), submit it to the airline for review and board the plane. Those who do not cooperate or meet the requirements will be refused boarding.
Third, how to choose the test time?
The validity period of health code is "sampling date +2 days".
1. For those who have been completely vaccinated with inactivated vaccine, please conduct two nucleic acid tests in two different testing institutions two days before boarding the plane (if the testing institutions are in different cities, upload the first test report to the relevant embassy or consulate with the city as the general). If the local testing organization does not have the conditions, it can also complete two nucleic acid tests within 3 days before boarding, but the interval between the two tests must be more than 24 hours, and the second sampling must be within 48 hours before boarding. People who do "double nucleic acid" test do not need to do serum (IgM) antibody test.
Note: Testing institutions refer to professional testing laboratories, non-domestic medical institutions and other testing acceptance or sampling institutions.
2. For those who have all been vaccinated with non-inactivated vaccine, please carry out nucleic acid detection and serum (IgM) anti-N protein antibody detection within 2 days before boarding the plane. The sampling date shown in the test report (if there is no clear sampling date in the report, the earliest date shown in the report shall prevail) shall not exceed 2 days from the boarding date of the flight to China. For example, if you want to take a flight on June 3rd, you can check it at June 1 at the earliest.

4. What are the requirements for the test report?
Only COVID-19 nucleic acid test (nucleic acid? Acid? Test) negative report and venous blood collection (venipuncture? Blood? Sampling), serum anti-N protein (nucleocapsid) IgM antibody negative report. For example, testing institutions provide whole serum antibodies (total? Antibodies (including IgM and IgG), the test results should be negative.
Please submit the standard original report issued by the testing organization. The report must include my name (consistent with passport and other travel documents), date of birth, testing (sampling) time, testing results and the name of the testing institution. If the original report of the testing institution cannot be issued, the report (including the inbox report) issued by the family doctor or medical institution can be submitted, and the name of the testing institution must be indicated, and the doctor's signature or clinic seal is required.
Do not accept the notification of test results with incomplete information such as short messages or test certificates without signature or seal.
5. What are the valid travel documents?
When applying for a health code, New Zealand and third-country citizens should submit their valid passport information, Chinese mainland residents should submit their valid China passport information, and Hong Kong, Macao and Taiwan residents should submit the corresponding information on the Pass for Hong Kong and Macao Residents to and from the Mainland, the mainland travel permit for taiwan residents or the Pass for China People to and from the Mainland.
The submitted travel document information must be consistent with the document number corresponding to the test report, vaccination certificate, residence certificate, travel itinerary and other application materials.
6. What if the residence certificate in New Zealand has expired?
(1) If you hold a New Zealand visa or residence permit that has expired for one year, please upload the expired visa and fill in and submit the detention statement (see Annex 4);
(2) If you hold a New Zealand visa or residence permit overdue for more than one year, please submit one of the following materials and fill in the detention statement (see Annex 4):

8. I have been vaccinated. Do you need a professional test before you go?
"Double nucleic acid" test or "nucleic acid+serum (IgM) anti-N protein antibody" test is still needed after vaccination. Please upload my vaccination certificate and test report truthfully.
9. Can I go to China without quarantine after vaccination?
At present, all people going to China need to undergo quarantine medical observation and related tests at the place of entry. The specific isolation time limit and detection type shall be subject to the interpretation of the prevention and control department at the place of entry.
X when should I upload the report and apply for a health code?
After obtaining the test certificate, you should submit the application through the corresponding platform as soon as possible, and pay attention to the audit results at any time. If the application is returned (shown in red), you should resubmit the supplementary materials as soon as possible.
Applicants are advised to submit their applications as soon as possible after preparing the relevant application materials. If the flight takes off in the early morning or morning, the application submission time shall not be later than 20: 00 the day before yesterday; For flights at other times, the application should be submitted no later than 5 hours before departure. In order to avoid missing the plane, it is strongly recommended to prepare the health code before arriving at the airport.

XI。 What should I pay attention to when reporting personal information?
1. When filling in personal information, China citizens (including Hong Kong, Macao and Taiwan residents) must fill in their Chinese names and ID numbers consistent with their valid travel documents, and Hong Kong, Macao and Taiwan residents should also upload photos on the information page of the documents they choose to declare.
2. Please provide New Zealand mobile phone number. If you don't have a local mobile phone, please make sure that the filled domestic mobile phone is unblocked so that the staff can contact the applicant urgently.
3. Please select the embassy or consulate to submit according to the consular district where the testing institution is located. The territorial division is as follows:

14. Does the baby need to apply for a health code?
Babies under the age of 5 (as of the day of departure) who take the direct flight from New Zealand to China have obtained a valid health code from their companion guardians, and should apply with negative nucleic acid reports detected within 2 days before boarding.
Fifteen, how to apply for the health code for the landing crew?
The crew members who are newly disembarked and intend to take a flight to return to China via Auckland must undergo nucleic acid pre-detection after disembarkation. After obtaining a negative report, they will undergo isolation observation and closed-loop management for 14 days. After the quarantine expires, apply for a health code with the Letter of Commitment for Closed-loop Management of Crew Quarantine (see Annex 3) issued by our company, and the "double negative" certificate of nucleic acid test and serum (IgM) anti-N protein antibody test within 2 days before boarding the plane (see Article 3 of this notice for testing requirements). Please ask the relevant crew, crew management company and shipping agency to know the entry quarantine policy of New Zealand in advance. If you can't meet the relevant requirements, please carefully choose to disembark in New Zealand.
16. People with past infection history
Those who have been diagnosed with COVID-19's disease, or those who have not been diagnosed but have any positive results of nucleic acid, IgM and IgG antibodies (except those who have been vaccinated with COVID-19 vaccine IgM and IgG antibodies) are regarded as having a past infection history in principle. After recovery, isolation and health monitoring should be completed, and health codes can only be applied after relevant inspections. Main process:

18. Can I transfer to China?
At present, the global COVID-19 epidemic is still very severe, and the risk of cross-infection in international travel is high. In order to avoid the risk of cross-infection caused by multi-country transit, people who go to China from New Zealand must fly directly to China, and are not allowed to transit China through a third country. People from other countries that have direct flights with China are not allowed to transit back to China via New Zealand.
