Acceptance time
The acceptance time of entry medical examination for overseas personnel is from Monday to Friday at 08:00- 1 1:00. In addition, according to the change of the number of physical examination, we will arrange physical examination for overseas personnel from Monday to Thursday afternoon and Saturday irregularly, and the specific arrangement will be subject to the appointment on the same day.
Medical appointment
The health assessment and physical examination of overseas personnel shall be carried out by appointment system. Please make an appointment at least two weeks in advance. There are two specific ways of appointment:
1. online reservation: make an appointment through this website.
2. I or my agent bring the identity certificate or copy (passport/Hong Kong and Macao Pass/Taiwanese Pass/Travel Permit) of the applicant for medical examination to make an appointment at the No.2 window of the public service center. On-site appointment acceptance time is from 8: 00 am to11:00 am from Monday to Friday, and from Monday to Thursday afternoon 13:00- 14:00 (except national holidays).
Carrier material
1. Application Form for Physical Examination (please print it in the "My Appointment" interface after online appointment, 2 pages in total).
2. Fully informed consent.
3. Identity documents:
(1) Foreigner: original passport+photocopy 1 (photo page+visa page/visa-free entry page+information change page).
(2) Hong Kong and Macao people: the original and photocopy of the Mainland Pass or Travel Permit for Hong Kong and Macao residents+1 copy (photo page+visa page/visa-free entry page+information change page).
(3) People from Taiwan Province Province: the original and photocopy of mainland travel permit for taiwan residents or Mainland Pass+1 copy (photo page+visa page/visa-free entry page+information change page).
(4) China citizens who have obtained permanent residency abroad: original passport+photocopy 1 copy (photo page+visa page/visa-free entry page+information change page).
4. Four two-inch color photos taken within half a year.
5. Relevant licenses:
(1) Work and residence: copy of business license or my employment certificate.
(2) Application for studying and staying abroad: copy of student ID card or admission notice.
(3) Accompanying family: a copy of the company business license and other relevant certificates of the accompanying staff.
(4) Residence for reasons such as visiting relatives, buying houses, treating diseases, getting married, returning to China for settlement, etc.: a copy of the temporary accommodation registration form issued by the public security organ or a copy of my real estate license and other relevant supporting documents.
6. If there are overseas health examination records within half a year, please bring the original health examination records issued by the hospital designated by the embassy of the host country (or region) for future reference, including the original laboratory test sheet and the original X-ray film.
Matters needing attention
1. Please go through the health assessment, physical examination and other procedures as soon as possible after entering the country, so as not to affect the subsequent visa processing.
2. Health assessment 5 hours before the physical examination, keep an empty stomach and drink water.
charging standards
The charge standard for the complete physical examination of overseas personnel is 58 1 yuan; Overseas applicants in Shanghai are doctors, and the full medical examination fee is 64 1 RMB; 466 yuan for international students; Additional inspections and services need to be charged separately. Please bring cash.
register for marriage
Permission is divided into on-site evidence collection and entrusted express evidence collection.
1. I took evidence at the scene. Persons who need physical examination shall take the receipt of evidence collection and the original valid identity certificate and go to the No.2 window of the public service center for evidence collection 4 working days after completing the physical examination.
2. Entrust the courier to collect evidence. After the physical examination, the medical examiner will handle the entrusted courier service himself, and the medical certificate will be delivered within four working days after the physical examination.
In order to ensure the safety and privacy of the physical examination results of the medical examiner, once the method of obtaining the certificate is determined, if it is necessary to change, the medical examiner himself should go to the No.2 window of the public service center to handle it.
Anomaly review
When abnormal conditions are found in health assessment and physical examination, the center will contact the candidates in time and issue a re-examination notice. Candidates should bring their original valid identity certificate and physical examination payment invoice to the designated department for re-examination within the specified time.
