Individuals declare themselves. The applicant shall truthfully fill in the Declaration Form of Health Pass for Returnees from Tangshan City, and the Health Pass will come into effect after the information integrity is reviewed by the community neighborhood committee.
"Health Pass" and "Tangshan Returnees Declaration Form" are supervised by the Municipal Prevention and Control Office, uniformly printed by the prevention and control offices of counties, districts and development zones, and distributed to towns and communities. "Health Pass" and "Tangshan Returned Personnel Declaration Form" can also be downloaded and printed by the applicant on the "Tangshan, China" website.
