Nursing diagnosis

Three statements of nursing diagnosis:

1, only p, used for health care diagnosis. Existing: Health data shows existing health problems. Potential: health data show that there are factors that endanger the nursing object, and problems that will occur if nursing measures are not taken.

The form of the statement is "danger". Possibility: there are suspicious factors, but there is no strong data support, or the relevant reasons are unknown. The form of the statement is: "It is possible ...". Health: It describes that individuals, families or communities have the potential to develop to a higher level of health. The presentation methods are: "potential promotion" and "effective implementation".

2. Two statements: PE formula. If the skin integrity is damaged, it is related to long-term bed rest.

3. Three statements: PSE formula: P (problem) is the name of nursing diagnosis, S (symptom or sign) includes the results of laboratory examination, and E (cause) is the relevant factor.

Criteria and factors of nursing diagnosis:

Criteria: It is the clinical judgment criteria for making this diagnosis. These standards are a sign, or a symptom, or a group of symptoms and signs, and may also be risk factors, and these standards are the reactions that individuals or groups actively express or observe.

This can be subjective or objective, and there are two kinds of subjective data and objective data: the main data must appear in the short-term diagnosis; Auxiliary data may appear during diagnosis.

Factors: refers to changes in health status or other problems caused by clinic or individuals. And these are usually "related" to nursing diagnosis.

Due to the natural differences and uniqueness of human individuals, the related factors vary from person to person due to different conditions. Relevant factors can be pathophysiology (biological or mental), psychology, treatment-related, situation (environment or individual) and maturity (i.e. age).
