Is there a time limit for driver's license to provide physical health certificate?

There is a time limit. Proof of physical condition can be submitted during the examination. It's not used now. Just fill out the form and truthfully declare your physical condition. This form should have been handed in then.

Motor vehicle drivers over the age of 60 shall have a physical examination once a year, and submit relevant physical condition certificates issued by medical institutions at or above the county level or at the regimental level within 30 days after the end of the scoring cycle.

Motor vehicle drivers who hold a driver's license for small automatic passenger cars for the disabled shall have a physical examination every three years, and submit a physical condition certificate issued by a specialized medical institution designated by the Provincial Health Department within 30 days after the end of the scoring cycle.
