Citizen health card loss reporting is divided into ordinary health card and multifunctional health card.
1. Ordinary health cards are not allowed to report loss. You can re-open a new general health card through the networked hospital in our city.
2. The balance of debit card account and card electronic cash account in multifunctional health card can be reported as loss, but card electronic cash account cannot. For specific provisions on loss reporting, please refer to the Administrative Measures for Great Wall Debit Card Business of Bank of China Limited.
(1) When the multi-functional health card is handled through the counter channel of China Bank, it is necessary to fill in the application form for reporting the loss of personal customers and provide the valid identity certificate of the cardholder, and at the same time, try to provide supplementary information such as card number, card opening date, card balance, reserved address and contact telephone number when opening the card. When you apply for card cancellation and temporary loss reporting, you must submit your debit card together with your valid ID card to the handling bank of China for relevant formalities.
(2) If the cardholder loses his multi-functional health card for various reasons, he can apply for a formal report of loss by debiting B, and he must fill in an application form for reporting the loss of individual customers, and provide the cardholder with valid identity documents to handle relevant formalities.
After the official loss reporting period expires, the card will be taken out, and the relevant procedures will be handled with the application for loss reporting by individual customers and the valid identity certificate of the cardholder. Card collection is not allowed.
In principle, the cancellation of the formal loss reporting of IC cards in real-name registration system should be handled by the cardholder with valid identity documents and the original application for loss reporting.
(3) After the official loss reporting expires, the prefabricated card can be replaced. You should hold an application for reporting the loss of an individual customer and provide the valid identity certificate of the cardholder to handle the relevant procedures for replacing the prefabricated card.
(4) If the card is cancelled after the official loss reporting period expires, an application for loss reporting by individual customers shall be held, and the cardholder's valid identity certificate shall be provided to handle the relevant card cancellation procedures.
