The adopter shall go through the adoption registration after being examined and filed by the adoption registration authority of the Civil Affairs Bureau.
2. The adopter shall provide the following supporting materials:
(1) death certificate of the adoptee's parents;
(2) the household registration book of the adoptee;
(3) The adoptee agrees (10 years old or older);
(4) the guardian's household registration book and ID card;
(5) the guardian's certificate;
(six) the certificate of consent of the dependents;
(seven) the adopter's household registration book and identity card;
(8) information such as the ability certificate of the adopter;
(9) The medical certificate of the adopter.

First, the adopter should have the conditions
1. The adopter should have a harmonious family, a stable job and good moral quality, and all family members have a good desire to adopt children.
Both husband and wife must be at least 30 years old.
3. The annual family income is more than 50,000 RMB and the living area is more than 80 square meters. The main family members have a high school education or above and have no criminal record.
4. Not suffering from diseases that are medically considered unsuitable for adopting children. (mainly mental illness and infectious diseases)
Mental illness includes schizophrenia, depression and other mental disorders.
Infectious diseases include Class A infectious diseases such as plague and cholera, Class B infectious diseases such as viral hepatitis, bacterial and amoebic diseases, typhoid and paratyphoid fever, AIDS, gonorrhea, syphilis, polio, measles, diphtheria, meningitis, scarlet fever, epidemic hemorrhagic fever and rabies, and Class C infectious diseases such as tuberculosis, leprosy and mumps.
Two, the adopter should provide documents and proof materials.
1, adoption application;
2, the adopter's identity card and residence booklet and a copy;
3, the adopter's unit or villagers' committee, residents' committee issued by the party's ideological quality, work performance, economic income, housing status, marital status, marital relationship, children's situation, raising ability and other certificates;
4. Provide the birth registration certificate issued by the family planning department of the place of residence, including the previous marriage and childbirth certificate;
5. A health certificate issued by a hospital at or above the county level designated by the adoption registration authority that has not suffered from a disease that should not be adopted in medicine should be attached with five hepatitis B detection documents;
6. Bank deposit certificate and certificate of no criminal record issued by the public security department;
7. Make two announcements in newspapers at or above the municipal level;
Third, the handling procedures
1. The adopter submits an adoption application to the foster care and adoption office of Qingdao Children's Welfare Institute with resident ID card, household registration book and marriage certificate, and fills in the adoption application form.
2, foster care, adoption office staff to make a preliminary understanding of the adopter's situation, and according to the situation of children in the hospital to decide whether to agree with the adopter's application.
For the applicant who agrees, the adopter shall provide the unit certificate, physical examination certificate and other relevant documents as required.
3, foster care, adoption office staff to review the materials provided by the adopter, fill in the "Qingdao Children's Welfare Institute to adopt abandoned baby approval form", put forward preliminary opinions, submitted to the hospital for review.
4. After the approval of the Children's Welfare Institute, the approval form, relevant certificates provided by the adopter and adopted children's files shall be reported to the Social Welfare Department of the Municipal Civil Affairs Bureau for approval.
5. The Children's Welfare Institute shall handle the adoption announcement at the adoption registration management office of the local district-level civil affairs bureau with the original approval form and supporting materials provided by the adopter.
6. After the expiration of the announcement, the adopter and the Children's Welfare Institute shall negotiate and sign an adoption agreement on adoption and donation.
7. The adopter shall go through the relevant formalities at the Adoption Registration Management Office of the District Civil Affairs Bureau with relevant approval materials and announcements.
