(2) The roster of employees of the employing unit shall be stamped with the official seal.

The steps for the unit to apply for a health certificate are as follows:
1. Show your ID card and business license to the staff, and let you fill out a form. After filling it out as required, you can get the anal testing tool.
2. Go to the corresponding floor to take a chest X-ray and draw blood as required. After the inspection, give the form to the staff.
3. Gently insert the anal test stick into the anus, rotate it clockwise for about 2-3cm, and then take it out. Give the anal test tube to the staff.
4. Five working days after the completion of the physical examination, bring a work certificate, two one-inch color photos and an ID card to receive a health certificate.
Matters needing attention in handling health certificate: take blood on an empty stomach before 9 am every day; Avoid strenuous exercise, drinking and eating meat before physical examination, so as not to affect the results of liver function test.
Health certificate refers to the proof of preventive health examination, which proves that the examinee has the health quality to engage in the prescribed business. Health certificate mainly involves six diseases in five industries, which largely protects the health of employees and clients. Health certificate refers to the pre-post, pre-post and age-appropriate health examination for personnel engaged in food and drinking water production and management, personnel directly engaged in cosmetics production, personnel who directly serve customers in public places, hazardous workers, radiation workers and school students according to the provisions of national health laws and regulations.

Article 5 The competent department of public places shall establish a health management system, equip full-time or part-time health management personnel, regularly check the health status of their business units (including individual operators, the same below), and provide necessary conditions.
Article 6 A business unit shall be responsible for the hygiene management of the public places it operates, establish a hygiene responsibility system, and conduct hygiene knowledge training and assessment for employees.
Seventh people who directly serve customers in public places must hold a "health certificate" before they can engage in their own work. Persons suffering from dysentery, typhoid fever, viral hepatitis, active tuberculosis, purulent or exudative skin diseases and other diseases that hinder public health shall not engage in direct customer service before they are cured.
