4. After entering the content to be searched, click the search button on the keyboard;
5. See the list of search results on the mobile phone page, and click the official account of Health Certificate WeChat;
6. Click the reservation option in the entered page;
7. You can choose the institution you want to apply for a health certificate to make an appointment.
Matters needing attention in handling health certificate:
1, ID card and 2 one-inch color photos of individuals bareheaded, receive the medical examination form, pay the medical examination fee at the toll booth, paste the photos (the other one is for health certificate), and fill in the basic information such as name and gender;
2. Blood collection should be carried out on an empty stomach before 9 am every day. Avoid strenuous exercise, drinking and eating meat before physical examination, so as not to affect the results of liver function test;
3. Time for obtaining evidence: at 2: 30 pm on the third day after the physical examination, take the invoice to the issuing window to receive the result of the permit (in case of physical examination on Thursday and Friday, it will be postponed to next Monday and Tuesday to receive the result of the permit);
4. The health certificate is valid for one year. Need to re-examine after expiration;
5. Those who need re-examination will go to the registration office of our department for re-examination at 8:30 am the next day on an empty stomach with their personal ID cards and invoices;
6. The physical examination results shall be kept for 3 months. Those who have not obtained a health certificate for more than 3 months will not be processed again, and they need to undergo a new physical examination, and then obtain a health certificate after passing the physical examination.
To sum up, health certificate refers to the certificate of health examination, which prevents books from being cancelled and proves that subjects have the health care and health preservation quality required by the industry. Health certificate mainly involves six diseases in five industries, which largely protects the health of employees and clients.
Legal basis:
Article 5 of the Regulations on Hygiene Management in Public Places.
The competent department of public places shall establish a health management system, equip full-time or part-time health management personnel, regularly check the health status of their business units (including individual operators, the same below), and provide necessary conditions. Article 6 A business unit shall be responsible for the hygiene management of the public places it operates, establish a hygiene responsibility system, and conduct hygiene knowledge training and assessment for employees. Seventh people who directly serve customers in public places must hold a "health certificate" before they can engage in their own work. Persons suffering from dysentery, typhoid fever, viral hepatitis, active tuberculosis, purulent or exudative skin diseases and other diseases that hinder public health shall not engage in direct customer service before they are cured.
