(3) Hearing disability
Various reasons lead to permanent hearing impairment in both ears to varying degrees, and they can't hear or hear the surrounding sounds and speech sounds clearly, thus affecting daily life and social participation.
Speech disability
Different degrees of speech disorders caused by various reasons, after more than one year of treatment or more than two years of course, make it impossible or difficult for them to carry out normal speech communication activities, thus affecting their daily life and social participation. Including aphasia, motor dysarthria, organic dysarthria, dysphonia, speech retardation in children, speech disorder caused by hearing impairment, stuttering, etc. Note: Persons under 3 years old are disabled indefinitely.
(5) Physical disability
The destruction of the structure and function of human motion system leads to the loss of human motion function to varying degrees and the limitation of activities or participation.
Physical disabilities mainly include:
(1) Loss, deformity or dysfunction of upper or lower limbs due to injury, disease or abnormal development;
(2) Spinal deformity or dysfunction caused by injury, disease or abnormal development;
(3) Trunk or limb dysfunction caused by central and peripheral nerve injury, disease or abnormal development.
(vi) Intellectual disability
Intelligence is obviously lower than that of ordinary people, accompanied by adaptive behavior disorder. This kind of disability is due to the structural and functional disorders of the nervous system, which limits the activities and participation of individuals and requires comprehensive, extensive, limited and intermittent support from the environment.
Intellectual disability includes mental retardation or mental retardation caused by various harmful factors in the process of intellectual development (18 years old); Or after the intellectual development is mature, due to various harmful factors, the intellectual damage or intellectual decline is obvious.
(7) Mental disability
Various mental disorders have not healed for more than one year, which affect daily life and social participation because of cognitive, emotional and behavioral obstacles.
(8) Multiple disabilities
At the same time, there are more than two kinds of disabilities: visual disability, hearing disability, speech disability, physical disability, intellectual disability and mental disability.
Chapter V, Classification of Disability
(A) the principle of grading
According to the degree of disability, all kinds of disabilities are divided into four levels, disability level 1, disability level 2, disability level 3 and disability level 4. The first-degree disability is extremely severe, the second-degree disability is severe, the third-degree disability is moderate, and the fourth-degree disability is mild.
(2) Classification of visual disability
According to the visual acuity and visual field, blindness is a first-and second-degree visual disability, and low vision is a third-and fourth-degree visual disability. Visual disability refers to both eyes. If the eyesight of both eyes is different, the one with better eyesight shall prevail. If only one eye has visual disability and the other eye has a visual acuity of more than 0.3, it does not belong to the category of visual disability. The visual field is centered on the fixation point, and the radius of visual field is less than 10 degree, regardless of visual acuity, it is blind.
(3) Classification of hearing disability
1, classification principle of hearing disability
According to the average hearing loss, the structure, function, activities and participation of the hearing system, environment and support, etc. (without hearing aid amplifier). Note: Children under 3 years old are defined as disabled if their degree of disability is one, two or three.
2. Hearing disability 1 grade
The structure and function of the auditory system are seriously damaged. The average hearing loss of a good ear is greater than 90 dB HL, so it is impossible to communicate by hearing. There are extremely limited activities in understanding and communication, and there are extremely serious obstacles in participating in social life.
3, hearing disability level 2
The structure and function of the auditory system are seriously damaged, with an average hearing loss of (8 1 ~ 90) dB HL, which seriously limits understanding and communication activities and has serious obstacles to participating in social life.
4, hearing disability level 3
The structure and function of the auditory system are moderately and severely damaged. The average hearing loss of healthy ears is between (6 1 ~ 80) dB HL, and the understanding and communication activities are moderately limited, and the participation in social life is moderately hindered.
5, hearing disability level 4
The structure and function of the auditory system are moderately damaged, and the average hearing loss of good ears is between (465,438+0 ~ 60) dB HL, and their understanding and communication activities are slightly limited, and their participation in social life is slightly hindered.
Classification of speech disability
1, classification principle of speech disability
According to the performance and degree of different types of speech disability, the structure, function, activity and participation, environment and support of brain and vocal organs.
2, speech disability level
The structure and function of the brain and/or vocal organs are seriously damaged, and there is no speech function or the speech intelligibility is less than or equal to 10%. The test of speech expression level can't reach the first level, and there are very serious obstacles to participate in social life.
3, speech disability level 2
The structure and function of the brain and/or vocal organs are seriously damaged, and they have certain vocal and speech abilities. The intelligibility of speech is between 1 1% ~ 25%, and the test of speech expression ability has not reached the level of the second test, which has serious obstacles to participating in social life.
4, speech disability level 3
The structure and function of the brain and/or vocal organs are moderately damaged, and some verbal communication can be carried out. The intelligibility of speech is between 26% and 45%, and the test of speech expression ability has not reached the level of level 3 test, so there are moderate obstacles to participating in social life.
5, speech disability level 4
The structure and function of the brain and/or vocal organs are slightly damaged, and simple conversations can be carried out, but it is difficult to express them in longer sentences. The intelligibility of speech is between 46% and 65%, and the test of speech expression ability has not reached the level of Band 4 test, which slightly hinders participation in social life.
(5) Classification of physical disabilities
1, classification principle of physical disability
According to the degree of motor function loss, activities are limited and participation is limited (no prosthetic limbs, orthotics and other auxiliary devices are worn). The limb part is described as follows:
(1) All upper limbs: including shoulder joint and scapula; (2) Upper arm: between elbow joint and shoulder joint, excluding shoulder joint, including elbow joint; (3) Forearm: between elbow joint and wrist joint, excluding elbow joint and including wrist joint; (4) All lower limbs: including hip joint and hemipelvis; (5) Thigh: between hip joint and knee joint, excluding hip joint and including knee joint; (6) Lower leg: between knee joint and ankle joint, excluding knee joint and including ankle joint; (7) Total finger loss: metacarpophalangeal joint; (8) Total toe loss: metatarsophalangeal joint.
2. Physical disability level
Can not independently realize the activities of daily life, and have one of the following conditions:
(1) quadriplegia: severe loss of motor function of limbs; (2) Paraplegia: complete loss of motor function of both lower limbs; (3) Hemiplegia: complete loss of motor function of one limb; (4) Loss of one upper limb and both lower limbs; (5) Loss of single lower limb and double forearms; (6) Loss of double upper arms and single thigh (or single calf); (7) Loss of both upper limbs or lower limbs;
(8) Loss of limbs in different parts above metacarpophalangeal joint (inclusive) and tarsometatarsal joint (inclusive); (9) Extremely severe dysfunction of both upper limbs or severe dysfunction of three limbs.
3, limb disability level 2
Basic can't independently realize the activities of daily life, and have one of the following conditions:
(1) Hemiplegia or paraplegia, the residual limb retains a little function (unable to walk independently); (2) Loss of both upper arms or forearms; (3) Loss of both thighs; (4) Loss of single upper limb and single thigh; (5) Loss of single lower limb and single upper arm; (6) Three limbs are missing in different parts above the metacarpophalangeal joint (inclusive) and tarsometatarsal joint (inclusive) of the foot (except for the first grade); (7) Severe dysfunction of both limbs or moderate dysfunction of three limbs.
4, limb disability level 3
Being able to realize daily life activities partially independently, and meeting one of the following conditions:
(1) Missing legs; (2) One forearm or more is missing; (3) Loss of more than one thigh; (4) Two thumbs or fingers other than two thumbs are completely missing; (5) Both limbs are missing in the metacarpophalangeal joint (inclusive) and different parts above tarsometatarsal joint (inclusive) of the foot (except the second grade); (6) Severe dysfunction of one limb or moderate dysfunction of both limbs.
5, limb disability level 4
Basically be able to realize daily life activities independently, and meet one of the following conditions:
(1) Single calf is missing; (2) Both lower limbs are unequal in length, and the gap is greater than or equal to 50 mm; ; (3) a strong (hard) straight spine; (4) Spinal deformity with kyphosis greater than 70 degrees or scoliosis greater than 45 degrees; (5) All fingers except one thumb are missing; (6) complete absence of one thumb; (7) tarsometatarsal joint is missing one foot above;
(8) The function of both toes is completely lost or lost; (9) dwarfism (adults whose height is less than or equal to 1300 mm); (10) Moderate dysfunction in one limb or mild dysfunction in both limbs; (1 1) Other limb dysfunction is the same as above.
6. Classification of mental disabilities
According to the developmental quotient, IQ and adaptive behavior of 0 ~ 6 years old and 7 years old and above. Children aged 0-6 are graded according to their developmental quotient directly if their developmental quotient is less than 72, and those whose developmental quotient is between 72 and 75 are graded according to their adaptive behavior. 7 years old and above are graded according to IQ and adaptive behavior; When the scores of the two are not at the same level, they are graded according to adaptive behavior. The score of WHO-DAS II reflects the activities and participation of people with intellectual disabilities at all levels, aged 65,438+08 and above.
7. Classification of mental disabilities
(1) classification principle of mental disability
18-year-old patients with mental disorders were scored according to WHO-DAS Ⅱ score and adaptive behavior performance, 18-year-old patients with mental disorders were scored according to adaptive behavior performance.
(2) the level of mental disability
The WHO-Das Ⅱ Ⅱ Ⅱ value is greater than or equal to 1 16, and the adaptive behavior is extremely serious. Life can't take care of itself at all, ignoring your basic physical and psychological requirements. Don't associate with people, can't work, can't learn new things. The environment needs to provide comprehensive and extensive support, and long-term life needs the care of others.
(3) Secondary mental disability
The value of WHO-DAS Ⅱ is between 106 ~ 1 15, which is suitable for severe behavioral disorders. Most people can't take care of themselves, basically don't communicate with others, only communicate with caregivers, understand the simple instructions of caregivers, and have certain learning ability. Able to do simple work under supervision. Can express their basic needs, occasionally passively participate in social activities. It needs the extensive support of the environment, and most of life still needs the care of others.
(4) Third-degree mental disability
The WHO-Das Ⅱ Ⅱ Ⅱ value is between 96 ~ 105, and the adaptive behavior is moderate. You can't take care of yourself completely in life. You can simply communicate with others and express your feelings. Can independently engage in simple labor, can learn new things, but the learning ability is obviously worse than the average person. Passive participation in social activities, occasionally active participation in social activities. The environment needs to provide some support, that is, the required support services are regular and short-term needs, and part of life needs to be taken care of by others.
(5) mental disability level 4
The WHO-DAS Ⅱ value is between 52 and 95, and the adaptive behavior is slightly impaired. Life is basically self-care, but self-care ability is worse than ordinary people, and sometimes personal hygiene is neglected. Being able to communicate with others, express their feelings and understand others' feelings is poor, and being able to engage in general work and learn new things is slightly worse than the average person. Sometimes, the environment is needed to provide support. Under normal circumstances, life does not need to be taken care of by others.
8. Classification of multiple disabilities
Determine the disability level according to the classification of the most serious disability category.

Referring to Article 3 of the Measures for the Administration of Disabled Persons' Cards in People's Republic of China (PRC), the disabled persons' cards adhere to the principle of voluntary application and territorial management. Referring to Article 5 of the Measures for the Administration of Disabled Persons' Cards in People's Republic of China (PRC), the number of disabled persons' cards is uniformly coded nationwide, and the first application adopts a 20-digit coding format, which is based on the citizenship number, disability category and disability grade code, and consists of 18 citizenship number plus 1 disability category code and 1 disability grade code. (1) Application: An applicant who applies for a disabled person's card for the first time must present the applicant's resident ID card, household registration book and three recent two-inch bareheaded photos with a white background to the county-level Disabled Persons' Federation where his household registration is located, and truthfully fill in the application form and evaluation form. When applying for a certificate of mental and mental disability and a minor's application for a certificate of disability, you must also provide proof of the legal guardian. Conditional places can apply online.
(2) Acceptance: After receiving the application materials submitted by the applicant, the county-level Disabled Persons' Federation will confirm the applicant, legal guardian, photo, ID card and household registration book, and those who fill in false information will not be accepted.
(3) Assessment: The designated institution will assess the disability of the applicant who applies for the Disabled Persons' Card, make a clear conclusion on the disability category and level according to the disability standard, fill in the assessment form and affix the official seal. If the assessment conclusion meets the disability standard, it shall be publicized in the village (community) where the applicant is located, and the publicity time shall be five working days; If the applicant is a minor, it shall not be publicized in principle.
(4) Examination and approval: The county-level Disabled Persons' Federation shall examine the application materials, acceptance procedures, disability assessment conclusions and publicity results, and complete the examination within ten working days.
If it meets the requirements after examination, it shall be approved, fill in and print the relevant information of the Disabled Persons' Card, affix the official seal to the column of the approved Disabled Persons' Federation, affix the steel seal to the head of the holder, and input the relevant information such as the Disability Assessment Form into the basic database of the disabled population.
If the appraisal conclusion does not meet the disability standard, it shall not be handled.
(V) Issuance and archiving: The county-level Disabled Persons' Federation issues disabled persons' cards to the applicants, and archives the application forms, evaluation forms, publicity results and other related materials for long-term preservation.
