1, copy of business license 1 copy;
2. Three copies of my ID card;
3. Two photos of yourself;
4. Copy of site certificate (provide a copy of my property right certificate, and the village committee that has not handled the property right certificate will issue a letter of introduction certificate. If you rent a house, you should provide a rental contract with the original owner's property certificate or a letter of introduction from the village Committee);
5. Original physical examination form (hospital at or above the county level);
6. A copy of the health certificate within the validity period (if there are employees, the health certificate should also be provided);
7. On-site inspection photo (four inches): an outdoor photo with a plaque and an indoor photo of myself standing at the counter.
Legal basis: Article 26 of the Administrative Measures for Food Business License. Food business operators shall properly keep the food business license, and shall not forge, alter, resell, lease, lend or transfer it.
A food business operator shall hang or place the original food business license in a prominent position in the business premises.
