The protagonist is a girl named Baby Doll. She is incarcerated in a mental institution by her demonic father, and within five days she will have her frontal lobe removed and become a permanent demented person. During her incarceration, she travels between her fantasized world and the real world.
A girl with a clear mind is forced to be incarcerated in a mental hospital, where her vicious stepfather plans to drive her insane in five days by various means. During this indecipherable period of time, the girl imagines a beautiful vision to numb the pain caused by life in the asylum: in that fantasy world, she begins to plan her escape in a league of green, but she must steal all five objects in order to get out of there; and if she doesn't hurry, she will be blasphemed by a despicable man five days later. During the night, the girl's mother dies and her will states that the inheritance is to be given to her two daughters. Her stepfather is furious and attacks the two sisters, but the sister fails to fight back with a gun and the bullet ricochets off her sister and kills her. The stepfather takes the opportunity to call the police and sends Doll to a mental hospital under the false pretense that he is depressed by the loss of her mother and has committed murder. Doll is taken to a "theater" in the asylum, where there are other girl patients and a therapist, Mrs. Gursky. Doll overhears her stepfather and nurse "Blue" talking behind her back about an illegal deal, bribing the nurse to forge Gorski's signature of consent for the surgery, which will be a lobotomy five days later to erase all of Doll's memories. Mrs. Gursky's therapy teaches the patients to take control of the world as they imagine it. Under the transformation, the asylum becomes a dancer's nightclub, where the stepfather becomes the priest who sells the orphaned dolls to the club owner, Blue Man, and Mrs. Gursky is the dance instructor who trains the girls. And five days later, the doll's virginity will be sold to the "Prodigal Son". On stage
Sweet Pea interrupts and criticizes a surgical performance. Her sister, Rocket Girl, familiarizes Doll with the theatre, and during an interlude, Doll saves Rocket Girl from the chef's lust, and the two become close friends. Mrs. Gossage begins her dance practice and orders Doll to dance to the music. As she dances, the doll transitions to the third level of imagination: a snowy Japanese dojo. The doll enters the dojo and meets the "Wise Man", who guides her to the key to escape, obtaining five items: a map, a fire, a knife, a key, and the fifth item is a riddle, which she has to figure out on her own. At this moment, three huge Japanese samurai attack, and the doll fights them off with the long sword given to her by the Wise Man. Back at the theater, the crowd applauds Doll's mesmerizing dancing performance, which is gradually gaining favor. Except for Sweet Pea, the girls all appreciate Doll's dancing performance. In the preparation room, Doll proposes a plan to escape, and Rocket and the other two girls, Black Amber and Boo Girl, follow, while Sweet Pea reluctantly agrees to the plan, stating that the plan will have to be aborted if it becomes too dangerous. The girls go into action, first copying the map placed in Blue's office. The doll dances again to get Blue to leave, and Sweet Pea takes the opportunity to sneak into the office and steal it. As the dolls dance, they switch back to a German WWI scenario, where the Wise Man instructs the girls to attack an enemy base in a trench to get the map. Amber pilots her flight armor to block the attack of the flying ships, while the other four attack the Steamers in the trench, and after a furious battle, the girls finally capture the map, and the dance ends. Delighted with the performance, the Blues explain to Mrs. Gossage that they want the dolls to perform for the Mayor, and get the money to sponsor the performance. Gusky thinks the performance is too crude, but Blue insists on rejecting it. Back at the office, Blue notices that the pin marks on the map have shifted. The girls quickly move on to their next plan, choosing Amber to steal the Mayor's lighter during the doll show in order to obtain the second item, fire. Amber approaches the Mayor to steal it while the Mayor is glued to the show. The show is set in a medieval fantasy world, and the Wise Men explain to the girls on the plane that the mission is to sneak into the castle and get the crystal stone from the throat of the baby dragon, which will create an unparalleled flame when the two stones hit each other. After successfully slitting the throat of the baby dragon, the girls are enraged by the dragoness, and the girls fight back, with the dolls finally succeeding in plunging their long swords into the dragon's head to kill it, and the show is over. After the girls celebrate the success of the two items at the backstage, the blue guy suspects that the girls are plotting behind the scenes and intimidates them. Afterwards, Sweet Pea decides to quit the show, and Boo Woman tells Mrs. Gossage about the plan out of fear, which is revealed to Blue. The other girls come to the kitchen to steal the chef's knife, but when the Boo-Girls don't show up, Sweet Pea returns to the group and the dolls start to dance to show the chef. The scene changes to a sci-fi future world where the girls must intercept a bomb code-named "Knife" in a high-speed train, which detonates as soon as the train enters the city. Amber pilots a helicopter, while Doll, Sweet Pea and Rocket Girl jump into the train and battle robot soldiers. When the bomb is finally deactivated, the radio's power cord is interrupted by a fire in the water, and the chef comes back to reality, attacking Sweet Pea with a knife in anger, and the music echoes. The train approaches the city and explodes, failing the mission. At the end of the show, Rocket Girl dies defending Sweet Pea from a knife attack. Angry Blue Man arrives and captures the girls, leaving Sweet Pea in captivity. Learning of all the plans, Blue Man shoots Amber and Boo Girl one by one and tries to sexually assault Doll, who at this moment pulls out the knife she obtained and stabs Blue Man, grabbing the key to escape and rescuing Sweet Pea. The two set fire to the room and escape with the map and key, but the door is guarded by many big men. Doll realizes that the fifth item is herself and must sacrifice herself to set Sweet Pea free. Finally, she walks out in front of the crowd, and when everyone notices her, Sweet Pea takes the opportunity to escape through the doorway, and Doll is knocked unconscious by a punch. Back in reality, the doctor is finishing a lobotomy on the doll. The doctor notices that the patient looks out of sorts, and Mrs. Gursky tells him that the patient has a bad record of arson, stabbing nurses, and letting a patient escape in the past five days, but still suspects her condition and discovers a forged signature. At the same time, the caretaker, Blue, is alone in the room with the doll and intends to sexually assault her, but to his horror, he shouts, "She's not there anymore", and Mrs. Gorski and the police discover the illegal transaction and arrest Blue. After the operation, the doll closes her eyes with a peaceful look as if she is free. When the escaped sweet pea is interrogated by two police officers who are looking for her, the bus driver, the Wise Man, relents, explains that the girl is her passenger and not the patient they are looking for, and takes the sweet pea to a faraway land of freedom.
