The protagonist is a girl named Baby Doll. She is incarcerated in a mental institution by her demonic father, and within five days she will have her frontal lobe removed and become a permanent demented person. During her incarceration, she travels between her fantasized world and the real world.

A girl with a clear mind is forced to be incarcerated in a mental hospital, where her vicious stepfather plans to drive her insane in five days by various means. During this indecipherable period of time, the girl imagines a beautiful vision to numb the pain caused by life in the asylum: in that fantasy world, she begins to plan her escape in a league of green, but she must steal all five objects in order to get out of there; and if she doesn't hurry, she will be blasphemed by a despicable man five days later. During the night, the girl's mother dies and her will states that the inheritance is to be given to her two daughters. Her stepfather is furious and attacks the two sisters, but the sister fails to fight back with a gun and the bullet ricochets off her sister and kills her. The stepfather takes the opportunity to call the police and sends Doll to a mental hospital under the false pretense that he is depressed by the loss of her mother and has committed murder. Doll is taken to a "theater" in the asylum, where there are other girl patients and a therapist, Mrs. Gursky. Doll overhears her stepfather and nurse "Blue" talking behind her back about an illegal deal, bribing the nurse to forge Gorski's signature of consent for the surgery, which will be a lobotomy five days later to erase all of Doll's memories. Mrs. Gossage begins her dance practice and orders Doll to dance to the music. As she dances, the doll transitions to the third level of imagination: a snowy Japanese dojo. The doll enters the dojo and meets the "Wise Man", who guides her to the key to escape, obtaining five items: a map, a fire, a knife, a key, and the fifth item is a riddle, which she has to figure out on her own. Amber approaches the Mayor to steal it while the Mayor is glued to the show. The show is set in a medieval fantasy world, and the Wise Men explain to the girls on the plane that the mission is to sneak into the castle and get the crystal stone from the throat of the baby dragon, which will create an unparalleled flame when the two stones hit each other. After successfully slitting the throat of the baby dragon, the girls are enraged by the dragoness, and the girls fight back, with the dolls finally succeeding in plunging their long swords into the dragon's head to kill it, and the show is over. The train approaches the city and explodes, failing the mission. At the end of the show, Rocket Girl dies defending Sweet Pea from a knife attack. Angry Blue Man arrives and captures the girls, leaving Sweet Pea in captivity. Learning of all the plans, Blue Man shoots Amber and Boo Girl one by one and tries to sexually assault Doll, who at this moment pulls out the knife she obtained and stabs Blue Man, grabbing the key to escape and rescuing Sweet Pea. The two set fire to the room and escape with the map and key, but the door is guarded by many big men. Doll realizes that the fifth item is herself and must sacrifice herself to set Sweet Pea free. Finally, she walks out in front of the crowd, and when everyone notices her, Sweet Pea takes the opportunity to escape through the doorway, and Doll is knocked unconscious by a punch. Back in reality, the doctor is finishing a lobotomy on the doll. When the escaped sweet pea is interrogated by two police officers who are looking for her, the bus driver, the Wise Man, relents, explains that the girl is her passenger and not the patient they are looking for, and takes the sweet pea to a faraway land of freedom.
