2. Bring your ID card and two one-inch bareheaded color photos, and receive the medical examination form;
3. Pay the medical examination fee at the toll booth, paste photos and fill in basic information such as name and gender;
4. Complete all physical examination items;
5. If the physical examination results meet the conditions for issuing a health certificate, a health certificate shall be issued; If the physical examination results do not meet the conditions for issuing a health certificate, a health certificate shall not be issued.
Legal basis: Article 45 of People's Republic of China (PRC) Food Safety Law.
Food producers and business operators shall establish and implement the health management system for employees. Persons suffering from diseases that hinder food safety as stipulated by the administrative department of health of the State Council shall not engage in direct contact with imported food.
Food production and marketing personnel engaged in direct contact with imported food shall undergo annual health examination and obtain health certificates before taking up their posts.
What items need to be checked for health certificate?
1, chest radiography, pneumonia and tuberculosis;
2, blood test alanine aminotransferase, if alanine aminotransferase increased more than twice, at the same time to check the hepatitis A antibody, hepatitis B five, hepatitis C antibody and other items;
3, internal medicine check blood pressure, check whether there are any major heart and lung diseases;
4, thyroid, breast, spine, limb joints and other surgical examination. ;
5, stool culture Vibrio cholerae, typhoid Bacillus, paratyphoid Bacillus, check whether there are the intestinal infectious diseases.
