What is the health certificate?
Health certificate is a health examination certificate, and it is a certificate that employees engaged in food production and management, public services, medical and health services and other industries must hold.
Generally speaking, the materials for handling health certificates are:
(1) Provide the original on-the-job certificates (such as work certificates) issued by public institutions, print A4 paper format, and affix the official seal of the unit (see sample table1for the style);
(two) employees in public places must provide a copy of the business license and health permit of the business unit, and must be stamped with the official seal of the unit (see Table 2 for the style);
(3) ID card and 1 recent color photos.
(Note: If the company has no official seal, it is required to provide a copy of the business license of the institution and the legal person ID card, and the legal person's signature and handprint are required on the sample form. )

Why do you need a health certificate?
Staff health examination mainly screens the following diseases: dysentery, salmonellosis, hepatitis A, hepatitis E, active tuberculosis, exudative or purulent skin diseases and other infectious diseases.
If you find these diseases, don't engage in the above five related jobs, such as directly serving customers, so as not to spread the diseases and endanger people's health.
Check again after the cure, and continue to engage in related work after passing the inspection.

Matters needing attention in physical examination
Eat normally on the day of physical examination, without fasting, and avoid taking drugs that affect liver function.
