(1) Original passport, Taiwanese certificate and Hong Kong and Macao pass;
(2) 3-5 recent 2-inch color ID photos;
Second, free infectious disease monitoring and physical examination.
Cooperate with free infectious disease surveillance and physical examination, and provide the original and photocopy of the following materials as required:
(1) foreign students: study visa and admission notice valid for the first time 1 year or above;
(2) Foreigners entering the country for employment and employment: the first valid work visa of 1 year or above, and the relevant certification documents of the foreign exchange bureau;
(3) Entry and settlement personnel: 1 year and above residence visa;
(4) Material verification of overseas medical institutions: original medical certificate, X-ray film (or CD);
(5) Outgoing laborers: working visas for more than one year, and certification documents issued by labor service companies;
(6) International students: visa for more than one year, overseas school admission notice, I20 form;
(7) Persons going abroad to visit relatives and settle down: visas for more than one year, invitations from destination countries and air tickets;
(8) China international transport workers leaving the country: international seaman's certificate, international seaman's visa of the relevant country, and proof of the unit where they work; Those who renew their health certificates need to carry the "Crew Health Certificate" and the physical examination report;
(9) China citizens who have lived abroad for more than 3 months and returned to China: those who have the seal of the frontier defense organ on their passports and have physical symptoms;
(10) Foreign language forms required by foreign countries (schools) (if necessary).
Legal basis: Notice of the Ministry of Health on health examination of some entry-exit personnel and issuance of health certificates for international travel.
First, the health examination object
(1) Personnel engaged in catering industry at frontier ports and on inbound and outbound means of transport;
(2) Traffic practitioners who frequently enter and leave the country;
(3) Persons who have stayed in foreign countries for more than 1 year or have gone abroad at home for more than 1 year, such as international students, advanced students, those engaged in business and technical cooperation, visiting relatives, exporting labor services, aiding foreign countries, etc.
Those involved in the above items (2) and (3) should do a good job in publicity and voluntarily accept medical examination.
Second, the health check items and specific measures
(1) Personnel and means of transportation in the food and drinking water industries at frontier ports shall be implemented in accordance with Article 19 of the Measures for Health Supervision at Frontier Ports;
(2) Check the personnel who have passed the physical examination according to the items specified in the Health Checklist (annex 1);
(3) Give priority to the physical examination and certification of transportation practitioners and passengers who want to leave the country in the near future, and make an appointment for inspection;
(four) after the completion of the health examination, the international travel health certificate shall be issued according to the examination results and the prescribed style (Annex 2). The certificate shall be carefully filled in according to the specified items, with neat and correct fonts and the photos stamped with the specified seal (Annex 3);
(five) those suffering from hypertension and heart disease can truthfully fill in the certificate. No health certificate will be issued to those who are found to have quarantinable infectious diseases, leprosy, open tuberculosis, sexually transmitted diseases, mental illness and mental retardation;
(6) A person who holds a medical certificate from a hospital at or above the county level and whose medical examination items meet the requirements may issue an international health certificate; If the project is incomplete, it should be replaced after inspection; Where a forged, altered or unqualified medical certificate is found, the International Travel Health Certificate will not be renewed;
(7) After the issuance or renewal of the International Travel Health Certificate, the physical examination form and laboratory report shall be kept in the archives of the health and quarantine organ. If the customer requests, you can send a copy to the customer and charge a certain handling fee.
