How to report the loss of China Bank Shenzhen Citizen Health Card?

The loss reporting of Shenzhen citizen health card of China Bank can be divided into two types: ordinary health card loss reporting and multifunctional health card loss reporting.

1. China Bank's general health card is not allowed to report the loss. You can re-open a new universal health card of China Bank through Shenzhen Internet Hospital.

2. The balance of debit account and card electronic cash account in the multifunctional health card of China Bank can be reported as loss, but card electronic cash account cannot. (1) When China Bank's multi-functional health card is debited to B card for temporary loss reporting through the counter channel, it is necessary to fill in an application for loss reporting by individual customers, and provide the valid identity certificate of the cardholder. At the same time, it is necessary to provide supplementary information such as China Bank's card number, card opening date, balance in the card, reserved address when opening the card, contact telephone number, etc. When applying for card cancellation and temporary loss reporting, you must go through the relevant formalities at the bank manager with China Bank debit card and valid identity documents.

(2) If the cardholder loses the Bank's multi-functional health card for various reasons, he can apply for a formal report of loss by debiting B, and must fill in the application for loss reporting of individual customers and provide the cardholder's valid identity documents for relevant procedures.

After the official loss reporting period expires, the card will be taken out, and the relevant procedures will be handled with the application for loss reporting by individual customers and the valid identity certificate of the cardholder. Card collection is not allowed.

In principle, the cancellation of the formal loss reporting of IC cards in real-name registration system should be handled by the cardholder with valid identity documents and the original application for loss reporting.

(3) After the official loss reporting expires, the prefabricated card can be replaced. You should hold an application for reporting the loss of an individual customer and provide the valid identity certificate of the cardholder to handle the relevant procedures for replacing the prefabricated card.

(4) If the card is cancelled after the official loss reporting period expires, an application for loss reporting by individual customers shall be held, and the cardholder's valid identity certificate shall be provided to handle the relevant card cancellation procedures.
