Take two one-inch color photos with ID card and personal bareheaded, pay the medical examination fee at the toll booth, get the medical examination form, paste the photos (the other one is for health certificate), fill in the basic information such as name and gender, and issue a health certificate after inspection. Health certificate refers to the proof of preventive health examination, which proves that the examinee has the health quality to engage in the prescribed business.
