How can I apply for a health card? 1. Open the Su Meng application in your phone. Before applying for a health card (health code), we need to complete real-name authentication, and the minimum real-name authentication level is level 2.
2. After real-name authentication, click on the health card at the top of the home page to view the resident health card;
3. Then on the resident electronic health card page, select the health card to apply for;
4. After entering the health card application page, the system will automatically receive an electronic health card for us, and click the health information declaration below;
5. Only after filling in the health information declaration form can you get the health code. Fill in the health card application page, enter your identity information and health status, and click Submit Application;
6. After submission, it will automatically jump to the health code page, and then we can display the health code through the health card on the resident electronic health card page (note that the health code needs to fill in the health status every day, because the health code is only valid for one day).
