Required information:
1. Original ID cards of both husband and wife.
2. The woman's household registration book.
3. A copy of the hospital B-ultrasound.

You'd better make a phone call before you go. If you don't handle the maternal and child health file in the morning, confirm the information you need to bring.
Fill in the system management records of pregnant and lying-in women: woman's name, age, occupation, education level, work unit, registered permanent residence, detailed address of current residence, contact telephone number, detailed address of postpartum training, number of pregnancies, last menstruation, expected date of delivery, gestational age, height, weight, blood pressure, blood type, and date of first hospital examination.
