"Health Pass" and "Tangshan Returnees Declaration Form" are supervised by the Municipal Prevention and Control Office, uniformly printed by the prevention and control offices of counties and development zones on the website of "Tangshan, China" and distributed to towns and communities. "Health Pass" and "Tangshan Returned Personnel Declaration Form" can also be downloaded and printed by the applicant on the "Tangshan, China" website.
