The materials required for claim settlement are as follows:
(1) Insurance claim;
(2) insurance policy;
(3) Identity certificates of the applicant and the insured;
(4) Important medical records such as diagnosis certificate, medical record, discharge summary (if there is hospitalization), medical expense invoice (including expense list) (including but not limited to hospitalization record (if there is hospitalization), temperature list, doctor's order list, laboratory test list (inspection report), medical image examination data, pathological data, nursing records, etc. ) issued by designated medical institutions and pharmacies.
(5) If compensation for related medical expenses has been obtained from basic medical insurance, free medical care, serious illness insurance, medical assistance and any third party (including any commercial medical insurance), provide the medical expenses split sheet or medical expenses settlement voucher of the basic medical insurance institution, commercial insurance institution or other third party;
(6) Other certificates and materials that the insured can provide to confirm the nature, cause and loss degree of the insured accident.
