On July 2 1, Fujian Food and Drug Administration issued the Notice on Implementing Online Registration of Licensed Pharmacists. The "Notice" clearly stated that from now on, licensed pharmacists can register online, and licensed pharmacists can log on to the licensed pharmacist registration platform and operate according to the requirements of the service guide.
According to the notice, in order to ensure the smooth transition of online approval of licensed pharmacists, Fujian Food and Drug Administration temporarily keeps the original online declaration window for a transition period of 6 months.
This is to give everyone a time to adapt, which also means that the offline window will be cancelled after half a year. Don't panic if a licensed pharmacist chooses the window to accept it. Now the offline window can continue to be accepted.
The specific operation of the whole online processing is:
1. Licensed pharmacists who need to register can log on to the licensed pharmacist registration platform and operate according to the requirements of the service guide.
2. Online declaration, window acceptance: online application-on-site acceptance-approval-certification (with licensed pharmacist qualification certificate by mail or window).
3. After applying for online registration and uploading the electronic license, send the materials required for different registration types (first time, change, re-registration and cancellation) to the accepting unit.
4. You can choose two ways to get the registration certificate: mail it home (get it) or pick it up at the window yourself.
5. The information must be accurate when mailing, and it is promised to be done within 10 working days.
6. When picking up the goods at the window, you need to bring your original ID card, and the agent needs to bring the original client qualification certificate and the original client ID card.

First registration of licensed pharmacists
1. Application Form for First Registration of Licensed Pharmacists;
2 "Licensed Pharmacist Qualification Certificate" (qualification certificate number and name are required);
3. Logo (front and back);
4. A health certificate issued by a disease prevention and control institution at or above the county level, or a physical examination form of a hospital at or above the county level;
5. The legal practice certificate of the practice unit (including the business license and the Pharmaceutical Production and Business License or the Medical Institution License);
6. The original electronic version of the recent two-inch bareheaded front half-length photo;
7. Continuing education credit certificate (all continuing education certificates before registration are required);
8. The licensed pharmacist himself holds a color photo of his ID card;

2. The continuing education of licensed pharmacists is subject to the credit system. Persons with the qualification of licensed pharmacists shall receive continuing education of licensed pharmacists for not less than 15 credits every year, and the accumulated credits within three years of registration shall be not less than 45 credits. Compulsory and elective contents shall not be less than 10 credits per year.
3. The Application Form for the First Registration of Licensed Pharmacists (attached with photos and stamped with the official seal of the practicing unit) and 1 recent two-inch bareheaded photos are sent to the licensed pharmacist registration window of the provincial bureau.
Re-registration of licensed pharmacists
1. Application form for re-registration of licensed pharmacists;
2 "Licensed Pharmacist Qualification Certificate" (qualification certificate number and name are required);
3. Logo (front and back);
4. Continuing education certificates (all continuing education certificates from the date of issuance of the original registration certificate to the future);
5. A health certificate issued by a disease prevention and control institution at or above the county level, or a physical examination form of a hospital at or above the county level;
6. Licensed pharmacist registration certificate; (You have gone through the cancellation procedures, just upload the original application form for cancellation of registration of licensed pharmacists);
7. The original electronic version of the recent two-inch bareheaded front half-length photo;
8. The licensed pharmacist himself holds a color photo of his ID card;

Change of registration of licensed pharmacists

4. Proof of continuing education between the previous registration and the application for change;
5. The legal practice certificate of the new practice unit (including Business License and Pharmaceutical Production and Business License or Medical Institution Practice License);
6. A health certificate issued by a disease prevention and control institution at or above the county level, or a physical examination form of a hospital at or above the county level;
7. Licensed pharmacist registration certificate;
8. The original electronic version of the recent two-inch bareheaded front half-length photo;
9. The licensed pharmacist himself holds a color photo of his ID card.
