3. Confirmed cases, suspected cases and asymptomatic infected persons treated in isolation in COVID-19, close contacts during centralized isolation and other personnel during medical observation shall not take the examination.
Candidates who have lived in high-risk areas within 0/4 days before the test and whose "health code" in Zhejiang is not green should provide negative proof materials of nucleic acid testing for nearly 7 days or show the green code of "health code" containing negative information of nucleic acid testing. If you can't provide the above proof, you must go to a comprehensive service point or a qualified testing institution for nucleic acid testing as soon as you come to Zhejiang. Only those who have negative results in nucleic acid testing can participate in the testing. If there are related symptoms (abnormal body temperature, dry cough, fatigue, sore throat, diarrhea, etc. ), must be treated in a designated hospital, and provide negative proof materials for nucleic acid testing twice within 7 days (more than 24 hours apart) before taking the exam. Candidates who were previously confirmed cases in COVID-19, asymptomatic infected persons and their close contacts should provide negative proof of nucleic acid test within 7 days before the exam.
4. Candidates must conduct health monitoring on their own from 14 days before the exam. If there is any abnormality, you should seek medical treatment in time and report to the education examination institution at the place of registration. Before taking the exam, all candidates must conduct self-screening according to the items listed in the Report on the Health Status of Candidates of Zhejiang Primary and Secondary School Teacher Qualification Examination (Written Examination) in 2020 (the annex can be downloaded). If there is any information listed in the column "Candidates voluntarily fill in and declare" (health code is not green code, symptoms have appeared within 2 weeks before the test, people who have been infected in the past, close contacts of infected people, people with epidemiological history within 2 weeks before the test, people who need to be tested for nucleic acid, etc. ), they should fill in truthfully, and print 1 to 3 copies according to the number of subjects they signed up for the exam, and each exam will enter the test center.
When entering the test center, if you don't voluntarily submit the Report Form on the Health Status of Candidates of Zhejiang Primary and Secondary School Teacher Qualification Examination (Written Examination) in 2020, it is deemed that the candidates have promised to read it carefully and agree to abide by the requirements listed in this notice, and know the responsibilities and legal consequences that they will bear if they conceal relevant information.
5. During the examination, candidates must abide by the relevant provisions of the examination institutions and test centers on epidemic prevention and control, actively cooperate with health code verification and temperature detection, and obey the relevant arrangements of the examination institutions and test centers in case of emergency. According to the requirements of epidemic prevention and control, no one is allowed to enter the test center except the candidates.
6. All candidates must bring their own disposable medical masks or surgical masks, and their identification shall not be affected by wearing masks. Candidates should wear masks, queue up in an orderly way during non-examination time, and keep a distance of more than 1 meter from each other to avoid getting together. Get into the habit of washing hands frequently and cover yourself with paper towels, handkerchiefs and sleeves when sneezing or coughing.
