1. Procedures and related materials for the medical certificate of the newborn delivered in a normal hospital: 1. Determine the name of the newborn (if the name is not taken, it cannot be processed). 2. Provide the following materials: birth medical records (provided by the delivery hospital); Application form for birth medical certificate (provided by the hospital and filled in by family members); Original and photocopy of both parents' ID cards; Reproductive health certificate (birth certificate); Maternal health book (card established by our hospital). 3. Registration time: every Monday, Wednesday and Friday (except holidays). 4. Others: The medical certificate of birth is a legal document. Once handled, the name of the newborn cannot be changed, and it needs to be handled by either parent of the newborn. If you apply for a certificate more than three months after birth, you must also provide the household registration certificate or household registration book of both parents.
2. Provisions on the handling of birth certificates of domestic midwives delivering babies: 1. When a newborn is delivered by a midwife who holds the technical qualification certificate of a domestic midwife, the domestic midwife shall issue a birth medical record containing the relevant contents of the birth medical certificate, and issue a birth medical certificate to the local maternal and child health care institution. 2. For newborns delivered by medical and health institutions that provide midwifery technical services without permission and those who have not obtained the technical qualification certificate of domestic midwives, the local maternal and child health care institutions shall issue birth medical certificates for them.
