Decoding conditions: 7 days of home isolation medical observation+decoding application form +3 times of negative proof of nucleic acid detection.
Situation 4:/kloc-Contacts or potential exposures of the confirmed cases or asymptomatic infected persons within 0/4 days.
Decoding conditions: 65438+ 04 days after the expiration of centralized or home isolation+medical observation certificate of release from isolation+decoding application form+negative certificate of 5 nucleic acid tests.
Situation 5:/kloc-No history of going out or contacting within 0/4 days.
Decoding conditions: personal commitment letter+decoding application form+a negative certificate of nucleic acid test.
Second, the operation process
1. Yellow code personnel can log in to the official WeChat account "Health 320" platform to submit complaints for decoding.
2. If the platform fails to appeal and meets one of the above decoding conditions, the individual shall provide proof materials such as health code, travel card and nucleic acid test certificate, and report to the local community and village committee.
3. The community or village committee shall conduct a preliminary examination of the materials submitted by individuals. If the decoding conditions are met, an application form for decoding shall be issued and stamped, and all materials shall be delivered to Hengdong County CDC after being summarized by the Township Prevention and Control Office, and the CDC shall review and change the code within two working days after receiving the materials.
4. In the future, if there is any adjustment to the provincial decoding operation flow, it shall be implemented according to the adjusted provisions.
