What documents should I bring with my health certificate?

Health certificate processing flow: 1, bring my ID card and two one-inch bareheaded photos, and go to the Municipal Center for Disease Control and Prevention to receive the physical examination form and training card, and fill in the basic information such as name and gender;

2. Blood collection and stool anal swab test in physical examination department;

3. Routine physical examination of surgery;

4. Complete all physical examination items and submit the physical examination form to the blood collection office;

5. Take a training card to participate in health knowledge training and assessment;

6. Those who pass the physical examination and training will go to the physical examination department of the handling center for health certificate after 7 working days.

Article 45

Food producers and business operators shall establish and implement the health management system for employees. Persons suffering from diseases that hinder food safety as stipulated by the administrative department of health of the State Council shall not engage in direct contact with imported food.

Food production and marketing personnel engaged in direct contact with imported food shall undergo annual health examination and obtain health certificates before taking up their posts.
