(1) On-day registration process (including Huaxi Health Dragon Card, Huaxi Health Pass Card, registered medical card and temporary medical card);

Tips:
(1) The medical card is only for the patient's own use, and it is not allowed to be lent, otherwise the consequences will be at your own risk;
(2) Medical card processing: (1) In Sichuan Province, Huaxi Health Dragon Card can be processed at the local Construction Bank, and Huaxi Health Smart Card can be processed at ICBC; (2) Temporary medical cards are handled in the lobby on the first floor of the outpatient building;
(3) The outpatient service time is 8: 00-11:45; 12: 30- 16: 00 in the afternoon, with an amateur clinic on Saturday and Sunday;
(4) Appointment time for taking the number: take the morning number before 9:30 am and the afternoon number before 65438+ 0:30 pm; If you don't get the number after making an appointment by phone or online, it will be regarded as a breach of contract. The system will automatically lock the reservation function of the card when the default number reaches three times, and then the card can only be registered on the spot on the same day. If you need to unlock, you need to bring your card and ID card to the outpatient clinic to register and verify the information six months before unlocking;
(5) With the doctor's consent, patients who return to the clinic can directly make an appointment for the next return visit (no more than 2 months) in the doctor's consulting room or nurse station;
(6) The doctor you made an appointment may stop seeing a doctor for some reason, and our hospital will arrange the corresponding doctor to see a doctor instead. If it is unacceptable to change the number, you can choose to return the number.
