(1) application
The applicant shall obtain an application form from the local quality and technical supervision department, fill in the application form in quadruplicate, and apply to the local quality and technical supervision bureau, and attach the following materials (one for each):
1. Copy of business license obtained according to law.
2. Copy of organization code certificate.
3. Quality manual.
4. Other materials specified in relevant safety technical specifications.
5. Equipment drawings and technical documents.
(2) Accept
The Provincial Bureau of Quality and Technical Supervision shall verify the application materials, and if it meets the requirements, it shall sign the acceptance opinion on the application form and return it to the applicant. Do not meet the requirements, to show the applicant a notice of rejection.
(3) Trial production and type test of products
After successful acceptance, the applicant company shall conduct product manufacturing or type test according to relevant requirements. After the completion of the type test, the unit undertaking the type test shall submit the type test report to the applicant.
(4) Inviting appraisal and evaluation institutions.
The applicant company shall bring the information of successful application and the type test report of relevant equipment, and invite the appraisal institution to conduct on-the-spot investigation and review.
When evaluating, the applicant enterprise shall conduct standardized operation in accordance with the production license and other safety technologies, and the evaluation institution shall issue an evaluation report to the Provincial Bureau of Quality and Technical Supervision after completing the evaluation.
(5) Examination, approval and certification
After passing the examination, the local provincial bureau of quality and technical supervision shall go through the examination and approval procedures. In line with the conditions, one original and one copy of the Special Equipment Manufacturing License will be issued. If it does not meet the requirements, a decision of disapproval shall be issued to the applicant company. Finally, the license will be issued to the applicant in the name of AQSIQ, so as to successfully apply for the special equipment manufacturing license!
