1. Written application report and application for the network of equipment, such as pictures, functional descriptions and relevant technical information.
2. Business license of enterprise legal person (copy). Sino-foreign joint venture or wholly foreign-owned enterprises need to provide the competent government departments to approve the ratio of domestic and foreign sales approval and joint venture contract, articles of association.
3. Opinions of the domestic production authorities (no production authorities should provide the appropriate effective documents).
4. General information about the enterprise (including personnel, capital, site, equipment, production conditions, affiliation, etc.).
5. Quality assurance system, after-sales service measures.
6. Mobile phones, cordless phones should also provide the local radio regulatory commission's approval documents.
7. "110" telephone alarm should also provide the Ministry of Public Security approved documents.

2. Enterprise legal person business license (copy).
3. The relevant state departments to authorize the import of certificates, customs declarations and duty-paid bills, if entrusted to other units of imports, should be accompanied by an agreement or contract between the two sides.
4. Application for access to the network equipment equipment serial number (except for mobile phones and wireless pagers), to be formally printed (in triplicate).
5. Application for access to the network equipment related information (including pictures of equipment, Chinese manuals, manuals, maintenance manuals, circuit diagrams, etc.).
6. After-sales service measures and to ensure the provision of maintenance of spare parts, spare parts of the certificate.
7. The distribution unit should provide the application for network models in the Ministry of Posts and Telecommunications designated testing unit to do the environmental adaptability test, life test and safety test report.
