Animal diagnostic and treatment institutions include animal hospitals and animal clinics.?
1, there are fixed animal diagnosis and treatment place, animal hospital area of not less than 120 square meters, animal clinic area of not less than 60 square meters;?
2, the animal hospital site should be no less than 200 meters away from livestock and poultry farms, slaughter and processing plants, animal trading places;?
3, animal clinics with independent entrances and exits, entrances and exits shall not be located in residential buildings or yards, and shall not be used with other users of the same building **** access;?
4, functional room settings and equipment is reasonable: animal hospital requirements are divided into diagnostic and treatment rooms, operating rooms, pharmacy, X-ray or ultrasound machine, etc., the animal clinic can not implement animal cranial cavity, thoracic surgery. With diagnostic, surgical, disinfection, refrigeration, routine laboratory, sewage treatment and other instruments and equipment;?
5, animal hospitals have more than three people who have obtained the certificate of licensed veterinary surgeons, animal clinics have more than one person who has obtained the certificate of licensed veterinary surgeons;?
6, with a perfect diagnosis and treatment services, epidemic reporting, sanitation and disinfection, veterinary prescriptions, drugs and harmless disposal management system.
Two, the procedure?
1, application. Application to the District Animal Husbandry and Veterinary Bureau, and submit the following materials:?
(1) animal clinic license application form (two copies);?
(2) animal clinic geographical location map, indoor floor plan and the layout of each functional area (two copies each);?
(3) proof of the right to use the animal clinic premises (review the original lease contract or real estate license, submit two copies);?
(4) legal representative (person in charge) identification (review the original, submit two copies);?
(5) the original and copy of the licensed veterinary surgeon's qualification certificate (submit two copies);?
(6) List of facilities and equipment (two copies);?
(7) Text of management system (two copies);?
(8) Health certificates of practicing veterinarians and service personnel (review the original and submit two copies).?
If the application materials are incomplete or do not meet the prescribed conditions, the District Animal Husbandry and Veterinary Bureau will inform the applicant of the contents to be corrected at once within five working days from the date of receipt of the application materials.?
2, District Animal Husbandry and Veterinary Bureau after acceptance, within 20 working days to complete the review of the application materials and the field examination of animal clinics. Meet the prescribed conditions, the applicant issued an animal diagnosis and treatment license; does not meet the conditions, notify the applicant in writing, and explain the reasons.
