Do you know what "the first gynecological tumor" is? Gynecologists pointed out that hysteromyoma is a big problem in many women's stomachs. According to statistics, about one-third to one-quarter of women suffer from hysteromyoma, and more than half of women aged 40 to 50 suffer from this disease, which is a common problem in gynecology. However, if the myoma is too large, it may cause frequent urination and excessive menstrual blood, which will affect the quality of life and seriously lead to infertility.
The hysteromyoma is too big, is hysterectomy the only choice?
Taiwan Province Medical Association of Gynaecology Haifu Therapy hosted a lecture on "Operation for Protecting the Palace without Knife-Haifu Knife". Zheng, the chairman of Taiwan Province Medical Association of Gynaecology Haifu Therapy and a professor of obstetrics and gynecology at higher medical university, appeared in the lecture, which attracted many mothers, daughters and sisters to come with questions and ask them enthusiastically.
Zheng pointed out that there are many methods to treat hysteromyoma, and the operation has developed from huge trauma and minimally invasive to non-invasive. HIFU is a non-invasive surgery, which focuses 60-90 degrees of heat energy on fibroids through high-energy focused ultrasound. Because human cells will die when they encounter a high temperature of 56 degrees, they can achieve the effect of melting fibroids, and the uterus can be preserved without surgery and wounds. Ablation is different from hyperthermia. The temperature of hyperthermia is only 40-55 degrees, and the tissue cannot be completely necrotic.
Zheng performed most gynecologic sea knife operations in South Taiwan Province Province, and most of them were used to treat hysteromyoma. He also found that many patients went south on purpose. Therefore, he went north to Boren Hospital in Taipei to cooperate with higher medical education and set up a twice-weekly Haifu treatment center, hoping that non-invasive medical technology could benefit more women.
Uterine leiomyoma is a benign tumor that grows from the myometrium of uterus. Lin Yushan, an attending physician in obstetrics and gynecology at Boren General Hospital, said that the reason for the formation is unknown. Female hysteromyoma increases in childbearing age and decreases after menopause. Therefore, judging female hormones is one of the causes of uterine fibroids.
Lin Yushan said that hysteromyoma can be divided into myometrial myoma, subserous myoma and submucosal myoma according to the growth position. Usually, when fibroids are large, organs near the uterus are compressed, and symptoms such as frequent urination and constipation appear, they will be found. Pay special attention to the symptoms of submucosal myoma, and it is best to check and follow up regularly every 3 to 6 months.
The malignant probability of hysteromyoma is about two thousandths, and the probability is extremely low. However, patients should still pay attention to the changes of symptoms, such as abnormal increase in bleeding, sudden growth of new fibroids, rapid growth of fibroids, and continued growth after menopause. Lin Yushan suggested discussing with the doctor whether to remove it surgically.
Zheng said that the treatment of uterine fibroids can temporarily shrink the fibroids by drugs, while traditional laparotomy, hysteroscopy, laparoscopy and da Vinci laparoscopic surgery can completely remove the uterus or remove the fibroids. However, for women with reproductive needs, laparotomy or endoscopic surgery may still cause infertility or adhesion problems.
The non-invasive operation of sea knife does not hurt other tissues, and the treatment time is about 1 to 2 hours. Severe sedation or sleep anesthesia can be used before operation, and the recovery time after operation is about 1 week, which can quickly improve the symptoms such as frequent urination and bleeding. After operation, the fibroids gradually disappear, and the volume will shrink by 40% in about 3 months, 50% in half a year and 85% in one year. But the symptoms improve quite quickly. Usually, after treatment, there will be obvious improvement in the next menstrual period, and a few will improve in the second and third cycles.
Zheng reminded that before treatment, patients need to do ultrasound examination, magnetic resonance vibration scanning and blood sampling to check tumor indicators to exclude malignant fibroids, which is suitable for sea knife surgery. According to the location and size of myoma, the effect is different. In addition, if you have uncontrollable hypertension, a history of myocardial infarction, severe arrhythmia, heart failure, renal failure and liver failure, it is not suitable for sea knife treatment.
Zheng added that women over 45 years old, who are close to menopause, do not want to leave scars on their stomachs due to surgery, or have recurrent uterine fibroids, and are afraid of general anesthesia, can consider the treatment of Haifu Knife. After accepting sea knife, it is still possible to relapse, so Lin Yushan suggested that everyone should pay attention to a balanced diet on weekdays, cultivate exercise habits and avoid getting sick.
What conditions are suitable for considering sea knife surgery?
■ Fear of anesthesia or surgery and minimally invasive surgery. ■ Close to menopause. ■ I hope there are no scars on my stomach. ■ History of hysteromyoma surgery. ■ Multiple fibroids or giant fibroids are not suitable for laparoscopic or Leonardo da Vinci surgery. ■ I hope to get pregnant in a short time without affecting the uterus. ■ People with poor liver and kidney function. ■ The purpose is to relieve or alleviate symptoms, such as dysmenorrhea, bleeding, frequent urination, etc. , and does not require the rapid disappearance of fibroids. ■ Worried that the operation will cause sticking.
