Applications are made by telephone after consultation with the patient's family to the hospice to apply and produce relevant information and fill out an appointment application form, the doctor will come to the door after the audit and establish a case file to prescribe medication and regularly carry out home visits, visits, diagnostic and treatment services.
Application requirements:
1, county (city) level or above hospitals to provide a confirmed diagnosis of the disease information (need to have the following two at the same time):
discharge summary or detailed outpatient records and related examination reports (including imaging, laboratory reports or pathological diagnostic reports, etc.); diagnosis of the disease certificate (stamped with the official seal of the hospital and need to receive the original for the record).
2. Information proving the family's economic poverty (one of the following is sufficient): low income guarantee issued by the civil affairs department, special hardship certificate for laid-off workers, etc. (the patient's name must be registered); poverty certificate issued by the neighborhood (village) committee (stamped with the official seal and the original must be received for record).
3. Valid patient's household registration, ID card and ID card of the person who takes medicine.
