Medical Certificate or Diagnostic Certificate Special (Disabled) Passenger Application for Flight Accompanying Person
South China Airlines only accepts applications for special services that are actually carried by South China Airlines and are on a South China Airlines flight number. If the flight involves other airlines other than South China Airlines such as connecting flights or return flights, you are requested to contact the actual carrier for the relevant procedures.
Passengers applying for special services may contact the ticket office directly under China Southern Airlines or call the Customer Service Center at 4006695539 for more information.
Medical Certificate or Diagnostic Certificate
1. China Southern Airlines reserves the right to request a passenger to present a medical certificate issued by a hospital recognized by China Southern Airlines and signed by the passenger's physician if the passenger has any of the following conditions:
a. A passenger traveling with a premature baby incubator;
b. A passenger who requires the use of medical oxygen equipment for oxygen infusion during the flight (at present, China Southern Airlines has not purchased and stocked on-board medical oxygen equipment except for oxygen cylinders for on-board emergencies, and does not provide on-board medical oxygen services for the time being);
c. A passenger whose health condition is endangered by traveling by air, or who needs special medical care;
d. a person who is known to be suffering from a serious contagious disease, but who can be prevented from infecting others if certain precautions are taken.
2. Medical certificate or diagnostic certificate in triplicate: the medical certificate or diagnostic certificate shall be filled in by the medical unit at county, city level or equivalent to this level with the passenger's condition and diagnosis, and signed by the doctor and stamped by the medical unit.
3. The medical certificate shall include the conditions that must be observed to prevent the spread of the disease or infectious disease, and in principle, it shall be valid only if it is filled out within 96 hours before the flight takes off, and in case of serious illness, it shall be filled out within 48 hours before the flight takes off. A medical certificate requiring a disabled person to prove that he/she can safely complete his/her trip without additional medical assistance during air travel shall be issued within 10 days prior to the date of departure of the flight.

Special (sick or disabled) passenger application form
Passengers applying for special travel services through China Southern's direct ticketing offices or customer service centers are required to provide a paper application form for air travel.
1. Passengers who are sick or disabled and request to fly on China Southern Airlines are required to fill out the "Application for Special (Sick or Disabled) Passenger Flight" in triplicate to indicate that the applicant assumes full responsibility for any aggravation of the passenger's condition, death, or injury caused to other people during the trip.
2. The "Application for Special (Sick and Disabled) Passengers" should be signed by the passenger himself/herself, or by his/her family member or guardian if he/she has difficulty in writing.
3. On China-U.S. routes operated by China Southern Airlines, China Southern Airlines shall not be required to waive its responsibility for the loss, damage, or death of wheelchairs or service animals checked in by passengers with disabilities.

Companion
1. The companion shall declare the companion relationship at the time of booking and shall be ticketed separately. The accompanying person shall travel on the same plane as the disabled person he/she accompanies. Passengers applying for WCHC/WCHS due to illnesses specified in Article 1 of the Passenger Travel Health Guidelines (except for passengers with disabilities) must be accompanied by a companion in the same physical cabin who is fully capable of civil behavior and has the ability to accompany them on the same flight.
2. The companion shall be capable of caring for the disabled person during the trip and assisting in his/her evacuation in case of emergency. Except for safety reasons, the seat of the companion shall be next to the seat of the disabled person.
Warm reminder: all kinds of special check-in business (including online check-in, SMS check-in, self-service check-in) does not accept special passengers and other check-in certificates for transportation of passengers (including important passengers, sick and injured passengers, disabled passengers, disabled military personnel (police), pregnant women, unaccompanied children, teenagers and the elderly, passengers needing special meals, etc.), please hold a valid identification document and other necessary documents provided at the time of purchase of the ticket. Please go to the check-in counter of China Southern Airlines directly with the valid ID and other necessary certificates provided at the time of purchase.
