Medical Thesis Opening Report

Medical Dissertation Opening Report Sample Template

Students are required to write a topic opening report before formally putting pen to paper to write their dissertation so that their supervisor can determine the feasibility of the topic based on the student's synthesis of the literature and knowledge of the chosen thesis. The following is a sample medical thesis opening report template that I have collected and organized for your reference and reference.

I. The significance of the topic and the current status of research at home and abroad

1. The significance of the selected topic

Chronic pulmonary heart disease, referred to as chronic pulmonary heart disease, is a structural and/or functional abnormality of the lung tissues caused by chronic diseases of the lung tissues, pulmonary blood vessels, or the thorax

Changes that produce pulmonary vascular resistance Increased pulmonary vascular resistance, increased pulmonary artery pressure, so that the right ventricle dilation and (or) hypertrophy, with or without right heart failure of a class of heart disease, and to exclude congenital heart disease and left heart lesions caused by the person. Coronary heart disease is a common and frequent disease in China. The census results in the 1970s showed that the prevalence of chronic pulmonary heart disease in the population aged >14 years was 4.8?[1]. According to the epidemiological survey, the incidence of pulmonary heart disease is high in China, with an average prevalence of 0.48% in the population, especially in the northeast and north China, and the percentages of pulmonary heart disease in various organic heart diseases are 18%~37% and 12%~34%, respectively. Most patients with pulmonary heart disease have a poor prognosis, with a case fatality rate of about 10-15%, and primary disease and respiratory failure are the main causes of death [2], which generally indicates that the prevalence rate is still high, and is still one of the main reasons for jeopardizing the life and health of human beings.

With the establishment of a sound social health insurance system, the rate of chronic pulmonary heart disease in primary hospitals

increased, so that the primary health care workers of this disease is more and more research. The principles of treatment for chronic pulmonary heart disease are to actively control infection; unobstruct the airway; improve respiratory function; correct hypoxia and carbon dioxide retention, control respiratory and cardiac failure; control heart failure; and actively manage complications. However, in the past, 'the main focus in controlling heart failure has been to enhance myocardial contractility, reduce cardiac load, and dilate blood vessels. In the enhancement of myocardial contractility, digitalis drugs applied to have some disadvantages, especially in patients with chronic pulmonary heart disease, often combined with electrolyte disorders, due to digitalis safety range is small, this situation is very easy to lead to digitalis drug toxicity, limiting the application of digitalis drugs; reduce the cardiac load, frequent diuresis easy to lead to sputum viscous, bring infection is not easy to control, asphyxia and other trouble; dilate blood vessels Drugs will lead to blood pressure instability, not conducive to the correction of heart failure. How to ensure that not only open airway, correct hypoxia, but also early control of heart failure to avoid further aggravation of the condition, which indicators can be prompted as early as possible the regression of our condition, to guide our treatment, to avoid excessive medical treatment, become an important topic of clinical treatment of chronic pulmonary heart disease.

Danshen Chuanxiongqiong injection is widely used in respiratory diseases, this study takes the patients diagnosed with chronic pulmonary heart disease in respiratory medicine as the research object, and adds Danshen Chuanxiongqiong injection on the basis of the conventional treatment, and observes and detects the changes of the three indexes of D-dimer, N-terminal brain natriuretic peptide, and troponin of the patients before and after the treatment, and evaluates the therapeutic effect of Danshen Chuanxiongqiong injection on the treatment of chronic pulmonary heart disease, which is important for the clinical treatment of chronic pulmonary heart disease. The effect of Danshen Chuanxiong injection on the treatment of chronic pulmonary heart disease is of great significance for the rational treatment of chronic pulmonary heart disease, improving the quality of life of patients, reducing the number of days of hospitalization, and decreasing the overconsumption of medical resources.

2, the status of domestic and foreign research:

Danshen Chuanxiongzine injection takes the motherland traditional Chinese medicine theory as the basis for the development of relatively little research in foreign countries. The drug is cheap and widely used. It has been reported in China that the addition of Danshen Chuanxiongzine Injection on the basis of conventional treatment has the effect of significantly improving blood viscosity, arterial blood carbon dioxide partial pressure, arterial blood oxygen partial pressure, hemoglobin, erythrocyte depressions, and pulmonary hemograms in patients with chronic pulmonary heart disease. Salvia miltiorrhiza and Chuanxiongxizine both inhibit platelet aggregation, dilate coronary arteries, improve microcirculation, and resist myocardial ischemia and myocardial infarction. Salvia divinorum can also regulate heart rhythm, improve the body's ability to resist hypoxia, anticoagulation, promote fibrinolysis, inhibit thrombosis; can reduce blood lipids, inhibit the formation of coronary atherosclerosis; can inhibit or alleviate hepatocellular degeneration, necrosis, and inflammatory response, promote hepatocyte regeneration, and anti-fibrotic effect. Chuanxiongzine has obvious sedative effect, while the respiratory center of the medulla oblongata, vasomotor center and spinal reflex center has an excitatory effect, and the aggregated platelets have a depolymerization effect, have to reduce blood viscosity, accelerate the role of red blood cell flow rate.

It has also been reported that D-dimer is a specific degradation product of cross-linked fibrin, and D-dimer in the blood is one of the markers specifically reflecting the hypercoagulable state and secondary hyperfibrinolysis in the body, and its increased level not only reflects the existence of secondary hyperfibrinolysis, but also indirectly reflects the enhancement of thrombin activity, which is valuable for diagnosis of hypercoagulable state in chronic pulmonary heart disease, and for observation of the efficacy of the treatment. It is of value for the diagnosis and observation of the efficacy of chronic pulmonary heart disease hypercoagulable state[3]. Plasma N-terminal brain natriuretic peptide is significantly elevated in chronic pulmonary heart disease in the period of decompensation, which is of some significance to the judgment of the condition of pulmonary heart disease, and it is a convenient, timely, accurate, and effective method for detecting acute congestive heart failure[4]. The obvious elevation of Troponin during acute exacerbation in patients with pulmonary heart disease is a reliable signal of critical condition, and the timely adoption of active and effective treatment measures is of great significance in reducing the morbidity and mortality of patients[5]. In this study, D-dimer, N-terminal brain natriuretic peptide and troponin were used to comprehensively assess the clinical efficacy of Danshen Chuanxiongzizine injection on chronic pulmonary heart disease, which was used to guide the clinical use of medication.

Second, the research objectives, research content and key problems to be solved

1. Research objectives:

(1) To clarify the effects of Danshen Chuanxiongzizine injection on D-dimer, N-terminal brain natriuretic peptide, and troponin in patients with chronic pulmonary heart disease.

(2) The significance of rational application of Danshen Chuanxiongzizine Injection on whether it can improve the patients with chronic pulmonary heart disease, quality of survival, prognosis, and optimize the allocation of medical resources.

2. Content of the study:

(1) Observe the improvement of patients' conditions before and after treatment: observe the indicators of HR, Rr, pH, CO2, PaCO2 (mmHg), PaO2 (mmHg), [HCO3-] (mmol/L), SaO2 (%).

(2) The biochemical indexes of plasma D-dimer, N-terminal brain natriuretic peptide and troponin were detected before and after the treatment of patients.

3. Key issues to be addressed:

(1) The compliance of the study participants in the treatment is the basic condition to ensure the integrity of the study. (2) The detection of patients' plasma D-dimer, N-terminal brain natriuretic peptide and troponin biochemical indexes, which can be detected in the respiratory department of the hospital, and the reduction of intermediate links in the delivery of specimens is the key to ensure that the information of the specimens is accurate and reliable.

Third, the proposed research methods, technical routes, experimental programs and their feasibility analysis

1, research methods:

① Inclusion criteria: Chronic pulmonary heart disease using the 6th edition of Internal Medicine, edited by Ye Rengao, Lu Zaiying? Pulmonary hypertension and pulmonary heart disease? The diagnostic criteria.

② research object collection January 1, 20xx - December 31, 20xx in our hospital hospitalization of all chronic pulmonary heart disease patients aged 40 ~ 90 years old, according to the principle of gender, age, disease with the principle of the research object is divided into: the research group (conventional treatment + Danshen Chuanxiong injection 10 ml intravenous drip), the control group (conventional treatment + Danshen Chuanxiong injection 5 ml intravenous drip). The control group (conventional treatment + danshen chuanxiong injection 5 ml IV drip). All of them are 1 time/day, 10-14 days as a course of treatment. Other treatments were the same in the study group and the control group.

③ Experimental process: plasma D-dimer, N-terminal brain natriuretic peptide and troponin were detected within 24 hours of admission to the hospital and one day before discharge from the hospital, and the detection instruments were FIA8000 immunoquantitative analyzer produced by Nanjing Prang Medical Equipment Co. All the study subjects completed the blood biochemistry examination according to the plan. Blood routine, liver function, kidney function, electrolytes, coagulation function, carbon dioxide, microorganisms and other blood biochemical indexes testing were completed in the laboratory of our hospital.

④ Data analysis of the study group, control group indicators for statistical analysis.

2. Technical route:

(1) Collect patients.

(2) Record data and organize information.

(3) Statistical analysis was performed using SPSS17.0 statistical software.

(4) Draw conclusions and write the paper

3, the test program: within 24 hours of admission to the hospital, the day before discharge were done plasma D-dimer, N-terminal brain natriuretic peptide and troponin detection, indicator detection equipment for: produced by the Nanjing Plan Medical Equipment Co., Ltd. FIA8000 immunoquantitative analyzers, specimen collection, the process of operation are undertaken by the researcher himself. All the study subjects completed the blood biochemistry examination according to the plan. Blood routine, liver function, kidney function, electrolytes, coagulation function, carbon dioxide, microbiology and other blood biochemical indexes testing were completed in our hospital laboratory.

4. Feasibility analysis:

(1) The number of patients with chronic pulmonary heart disease in winter is large, the case data is easy to collect, and Danshen Chuanxiongzizine injection is widely used in the clinic.

(2) The respiratory department of our hospital is able to independently complete the detection of plasma D-dimer, N-terminal brain natriuretic peptide and troponin within the department, and can easily obtain the experimental results, and the remaining relevant indicators can also be obtained from the hospital laboratory. Therefore, the feasibility of selecting the topic is relatively strong. The research of the subject is strongly supported by the department, and we believe that the subject can be successfully completed.

Four, the innovation of the subject

Danshen Chuanxiongzizine injection is widely used in respiratory medicine, the drug to improve the patient's microcirculation has been more research. However, in previous studies, fewer biochemical indicators were detected, or fewer indicators were detected, which could not accurately assess the patient's condition and the efficacy of the drug, and could not be used to guide the clinical use of drugs in a timely manner. Therefore, in this study, we used the effect of Danshen Chuanxiongzizine injection on plasma D-dimer, N-terminal brain natriuretic peptide and troponin in patients with chronic pulmonary heart disease, in order to objectively assess the clinical efficacy, timely guide the clinical treatment of chronic pulmonary heart disease, improve the quality of life of patients, reduce the number of days of hospitalization, and reduce the unnecessary waste of medical resources.
