Residents who have reached the age of 16 should bring their household registration book (permanent resident registration form should be submitted for collective accounts) and resident identity card (or temporary id card).

Students who have no military status in military academies must also submit the certificate of no military status issued by their colleges.
Soldiers who carry the military officer's card (or discharge certificate) and the original "Examination and Approval Form for Going Abroad for Private Purposes" shall be given opinions and stamped by their units on the "Examination and Approval Form for Citizens Going Abroad for Private Purposes", and the person in charge shall personally sign for confirmation.

Where a registered national staff member applies for a passport for personal reasons, the unit must issue an opinion on the Examination and Approval Form for China Citizens Going Abroad for Personal Reasons, with the signature and official seal of the person in charge. The specific personnel scope is as follows:
(1) Party and government organs at all levels, people's congresses, CPPCC, people's courts, people's procuratorates, people's organizations, institutions, leading cadres at or above the county (department) level, and retired cadres at or above the department (bureau) level;
(2) Financial institutions, legal representatives of state-owned enterprises, leading members at or above branches (branches) of financial institutions and leading cadres at corresponding ranks, middle-level and above managers of large and medium-sized state-owned enterprises, and representatives of state-owned shares of state-owned holding enterprises;
(3) Soldiers of China People's Liberation Army and People's Armed Police Force (including retirees);
(four) personnel involved in national security, state-owned assets security and industry secrets in various departments and industries;
(5) Other personnel registered by the exit-entry administration department of the public security organ.
Note: When applying for entry and exit documents, temporary resident identity cards should be submitted during the collection, renewal and replacement of resident identity cards.
2. To apply for studying in Malaysia, you need to bring the above materials to the Exit-Entry Administration Office of the Public Security Bureau where your household registration is located, fill in the Approval Form for China Citizens to Apply for Going Abroad for Private Purposes, copy relevant supporting materials and take photos (you must wear dark clothes).
Note: The Approval Form for China Citizens to Apply for Going Abroad for Private Purposes can also be downloaded, printed and filled in on the website of the Exit-Entry Administration Office of the Public Security Bureau. Please print it on both sides.
Third, submit an application.
Submit the completed application form together with photos and required materials to the acceptance window. After the review, the staff will receive the receipt of the application for going abroad for personal reasons, check the contents of the receipt and sign it.
Fourth, payment
After submitting the application, the applicant studying in Malaysia must pay the fee at the toll office with the Application Form for Going Abroad for Private Purposes.
(The applicant must pay the fee on the day of accepting the application. If the payment is overdue, the date of receiving the certificate will be notified separately. If the payment is not made within one month after the application, it will be deemed as a waiver of the application, and the application materials will not be returned to me)

Sixth, get a passport.
Personal collection: the applicant must collect the documents on time according to the date of evidence collection indicated on the receipt of the application for going abroad for private reasons or the date of evidence collection notified by the exit-entry administration department. On the day of obtaining evidence, the applicant himself will show the receipt of the application for going abroad for personal reasons and the receipt of payment, and bring the resident ID card or household registration book to the entry-exit reception hall where the application is accepted to collect the documents. After receiving the certificate, please carefully check the contents of the certificate. If there are any mistakes, please correct them in time.
On behalf of others: the person on behalf of the leader will bring the Receipt of Application for Going Abroad for Private Business, my ID card and a copy of the passport applicant's ID card to the Exit-Entry Administration Office to collect the passport.
Courier to your door: If you need to choose courier to your door, you must go through the formalities and pay the courier fee at the postal courier counter of the entry-exit administration office on the day of passport application with the Application for Receipt of Going Abroad for Private Business. The express delivery scope is subject to the regulations of the local entry-exit administration office.
Before applying for a visa, please sign your name in the signature column of the holder on the last page of your passport with a black pen.
Eight. Passport validity period
