Outpatient medical institution license for the required materials are as follows:

1, medical institutions, legal representatives or the main person in charge, as well as the head of each section of the directory and the relevant qualification certificates, practicing certificates, ID card copies;

2, medical institutions, property rights or proof of use of the premises;

3, the medical institution architectural design plan;

4, proof of capital verification, asset evaluation report;

5, list of general equipment and list of types of drugs.

Units or individuals applying for medical institutions should have the following basic conditions:

1, in line with the city's medical institutions set up planning;

2, in line with the norms of medical institutions set up in Xinzheng City;

3, the distance between hospitals and similar medical institutions is not less than 2 kilometers, the distance between outpatient clinics and similar medical institutions is not less than 1 kilometer, and the distance between clinics and similar medical institutions is not less than 0.5 kilometers. Distance of not less than 0.5 kilometers;

4, the total investment of not less than 50,000 yuan for clinics, not less than 100,000 yuan for outpatient clinics, and not less than 200,000 yuan for each bed in hospitals; and registered capital to meet the needs of practice.

Article 10 An application for the establishment of a medical institution shall be submitted with the following documents;

Organs, enterprises and institutions set up for internal staff services clinics, clinics, health centers (rooms) practice registration by the county-level people's government of the local health administrative department.
