The formalities, materials and related documents required to do business
1, a copy of the notice of approval of industrial and commercial name;
2, fire safety opinion of the business premises;
3, environmental protection department approval opinion;
4, the application report (print);
5, a copy of the ID card of the legal representative (outside of the Changsha Metropolitan Area need to be Migrant population family planning certificate);
6, the legal person without a criminal record of written proof;
7, the legal representative of the 1-inch photo;
8, business premises lease agreement or property rights certificates (copy);
9, the business premises geographic location and floor plan;
10, business premises regulations;
11, the relevant entertainment equipment information;
11, related entertainment equipment and equipment information and proof of bills.

According to the provisions of the Measures for the Administration of Entertainment Places, the current hardware requirements of the cultural department for the location of entertainment venues for singing and dancing are that they shall not be established in the following locations:
(a) buildings whose housing use includes residential;
(b) museums, libraries, and buildings that are approved for cultural relics protection;
(c) buildings that are approved for cultural relics protection; and buildings;
(iii) residential areas;
(iv) around primary and secondary schools as stipulated in the Education Law;
(v) around hospitals that have obtained a License to Practice in Medical Institutions in accordance with the Regulations on the Administration of Medical Institutions and the Implementing Rules;
(vi) around the committees of the Chinese ****anufacturing Party at all levels and their respective working departments, the organs of people's congresses at all levels, people's governments at all levels and their respective organs of the People's Congress, people's governments at all levels and their respective working departments, organs of the Political Consultative Conference at all levels, organs of the People's Courts and Procuratorates at all levels, and organs of democratic parties at all levels;
(vii) crowded places such as train stations and airports;
(viii) the first floor below the basement of a building (excluding the first floor below the basement);
(ix) warehouses of hazardous chemicals in the areas adjoined to the (ix) areas adjacent to dangerous chemical warehouses, and the distance from dangerous chemical warehouses must comply with the relevant provisions of the <
