How to fill in the registration form of medical insurance in different places

1. Please ask the retirement management department to receive the "Application Form for Medical Insurance in Different Places" in triplicate (the specific name varies from place to place) and affix the corresponding official seal.

2. Find three hospitals of different levels near where you live, namely (1) community hospitals; (2) General hospitals below 3A; (3), 3A hospitals; Go to these hospitals to find the medical insurance management department. (If you are in a big hospital, just ask the front desk in the lobby. If you are in a community, just ask the doctor in charge. ) Fill in three forms according to the information of different hospitals, and affix the official seal that you agree to accept in the corresponding column after being audited by the hospital.

3. Mail the form stamped with the official seal of the hospital to the department receiving the form of the Reclamation Bureau, and the following matters shall be handled by the staff. If they don't handle it, you can go to the local social security bureau for the record.
