In case of an affected person, the child's relationship to the legal guardian should be proved. Documents proving the relationship of the legal guardian;
Application form for Huma's Angel Fund funding filled out by the patient or the child's legal guardian;
Patient's preliminary medical examination report from a medical institution at or above the county level;
Proof of the family's economic status issued by an administrative institution above the town street office for a child with cleft lip and palate from an impoverished family of less than 2 years old who is a holder of a non-agricultural hukou.
