Personnel participating in the physical examination are applicants for teacher qualifications who have passed on-site confirmation by the Hohhot Municipal Education Bureau.
Applicants can log in to the "China Teacher Qualification Network" to check whether they have passed on-site confirmation. Enter from the "Teacher Qualification Certification Applicant Online Application Portal" and click the "Query Registration Information" column of "Teacher Qualification Certification Information". If the "Certification Status" shows "Pending Certification Approval", it means that the applicant has passed the on-site confirmation.

3. Make an appointment for a physical examination
To avoid gatherings, applicants must scan the following QR code to make an appointment. The physical examination can be performed only after the appointment is successful. Once an appointment is made, a physical examination must be performed within the specified time.
(1) Applicants who have been under closed management or quarantine are not allowed to participate in the physical examination; applicants whose body temperature exceeds 37.3°C, or whose "health code" is "yellow code or red code" are not allowed to participate in the physical examination.
(2) Physical examination personnel must bring at least 2 masks by themselves. In addition to taking off masks as required when entering the medical examination hospital for identity verification and physical examination-related items, they must wear masks throughout the process of entering and leaving the hospital and during other physical examination items to provide personal protection.
(3) Those who do not wear masks or have symptoms such as fever and cough are not allowed to enter the medical examination hospital. Physical examination personnel must follow the staff's arrangements, maintain a one-meter social distance, and comply with the epidemic prevention measures of the medical examination hospital. If the body temperature detected by the physical examination personnel is normal, but if abnormal conditions such as dry cough, shortness of breath, runny nose, diarrhea, etc. occur, they must proactively report to the hospital staff.
(4) Anyone who conceals or lies about key epidemic prevention and control information such as travel history, contact history, health status, etc., or fails to cooperate with staff in epidemic prevention testing, questioning, and personal protection, causing serious consequences, will be disqualified. If there is any illegal behavior, legal responsibility will be pursued in accordance with the law.
5. Things to note
(1) Physical examination personnel must bring their ID cards and 1 recent one-inch, half-length, full-frontal color photos with white background (the same as the electronic photo uploaded for registration, and their name and ID number must be marked at the bottom of the photo), and present them at the entrance of the physical examination institution "Qingcheng Medical" green health code, appointment SMS and cooperate with staff to measure body temperature before entering. The physical examination form is provided by the physical examination hospital.
(2) The physical examination must be done on an empty stomach. Do not drink alcohol or eat greasy or high-fat foods before the physical examination. If you have any special health conditions, please explain the reasons to the medical staff. The physical examination personnel will receive the physical examination form at the front desk, paste their photos and fill in the personal information on the form. After completing all physical examination items, submit the physical examination form to the front desk and leave. The medical examination fee shall be paid by the medical examiner to the medical examination hospital.
(3) After the physical examination, the physical examination hospital will submit the physical examination results to the Hohhot Education Bureau. Applicants cannot collect them by themselves. Those who fail the physical examination will not be recognized; if a re-examination is required after the physical examination, the hospital will notify the patient and the medical examiner must cooperate.
(4) The basic quality and ability test of education and teaching will be conducted in early February. The specific time and location will be notified later. Applicants are asked to continue to pay attention to the "Hohhot Education Bureau" and "Hohhot Education" public accounts.
