Application for the establishment of medical institutions or individuals to the county health bureau to submit the "medical practice license" application report, and fill out the "medical practice registration book" should be submitted: 1, the legal representative of the medical institution or the main person in charge, as well as the directory of heads of departments and relevant qualification certificates, practicing certificates, ID card copies; 2, medical institutions Proof of property rights or proof of use of the premises; 3, the architectural design plan of the medical institution; 4, proof of capital verification, asset evaluation report; 5, a list of general equipment and a list of types of drugs. Second, to apply for registration of medical institutions shall have the following conditions: (1) have set up medical institutions approval; (2) meet the basic standards of medical institutions; (3) have a suitable name, organization and premises; (4) have its business to carry out the appropriate funding, facilities, equipment and professional and technical personnel; (5) have the appropriate rules and regulations; (6) be able to independently assume civil liability.
Legal Objective:According to Article 19 of the Regulations on the Administration of Medical Institutions, the health administrative department of the local people's government at or above the county level shall conduct an audit in accordance with the Regulations and the basic standards for medical institutions within 45 days from the date of acceptance of an application for registration of practice. If the audit is qualified, it shall be registered and issued a "license to practice medicine"; if the audit is unqualified, the applicant shall be notified in writing of the results of the audit.
