1. Conditions for obtaining a business license:
(1) Shareholders meet the quorum;
(2) The capital contribution subscribed by all shareholders in accordance with the Articles of Association;
(3) Shareholders * * * agree to formulate the Articles of Association;
(4) Having a company name and establishing an organization meeting the requirements of a limited liability company;
(5) Having a company domicile.
2. Go through the business license formalities:
(1) An application for registration of establishment signed by the legal representative of the company;
(2) Certificates of the designated representatives or entrusted agents of all shareholders;
(3) Articles of association;
(four) the qualification certificate of shareholders or the identity certificate of natural persons;
(5) Documents stating the names and domiciles of the directors, supervisors and managers of the company, and certificates of appointment, election or employment;
(6) The post-holding documents and identity certificates of the legal representative of the company;
(7) Notice of pre-approval of enterprise name;
(8) Certificate of company domicile.
Three, for medical institutions practicing license conditions:
(1) Having passed the examination of professional skills of doctors and obtained the practicing certificate of doctors;
(2) Having been engaged in clinical work in the same profession for more than five years after obtaining a doctor's practice certificate or a doctor's title;
Four, for medical institutions practicing license procedures:
(a) the approval of the establishment of a medical institution or the receipt of the establishment of a medical institution;
(two) the certificate of ownership or use of the house used by the medical institution;
(3) Architectural design scheme of medical institutions;
(4) capital verification certificate and asset evaluation report;
(5) Rules and regulations of medical institutions;
(six) the name list of the legal representative or principal responsible person of the medical institution and the person in charge of each department and a copy of the relevant qualification certificate and practice certificate.
Apply to the local administrative department for industry and commerce for a business license, and apply to the local health administrative department at or above the county level for a medical institution practice license.

Twelfth any of the following circumstances, shall not apply for the establishment of medical institutions:
(a) units that cannot bear civil liability independently;
(2) An individual who is serving a sentence or does not have full capacity for civil conduct;
(3) Medical personnel who have suffered a medical accident of class II or above for less than five years;
(4) Medical personnel whose practice certificate has been revoked due to violation of relevant laws, regulations and rules;
(5) The legal representative or principal responsible person of the medical institution whose Practice License for Medical Institution has been revoked;
(6) Other circumstances stipulated by the administrative departments of health and family planning of the governments of provinces, autonomous regions and municipalities directly under the Central Government.
Under any of the circumstances listed in Items (2), (3), (4) and (5) of the preceding paragraph, the legal representative or principal responsible person of a medical institution shall not be appointed.
