Nursing duty and shift handover management system
This system specifies the work requirements for nursing staff on duty and shift handover. It is used to control the process of duty and shift handover for all nurses in order to ensure that nursing staff participate in duty and shift handover according to the specified requirements.
Responsibilities:
1. The Nursing Department is responsible for the development of the "nursing duty, shift handover management system".
2. The head nurse is responsible for the implementation of the system of random checks.
3. The head nurse participates in the shift handover and is responsible for checking, guiding and supervising the implementation of the system.
4. Nurses strictly implement the relevant regulations and conscientiously fulfill their duties.
Second, content and requirements:
1. duty:
(1) separate duty personnel should be registered nurses; nurses are not allowed to work alone.
(2) Each ward has a 24-hour duty officer. The staff on duty must fulfill their duties conscientiously and fill in the duty record.
(3) The staff on duty shall not leave the post without handing over the shift.
(4) on-time rounds, grasp the condition, find changes in the condition of timely report to the doctor on duty.
(5) Responsible for the disposal and resuscitation of new admissions, emergency patients, and critically ill patients in the ward.
(6) Completes the work of the shift on time and accurately, and guides the work of interns, trainee nurses and hygienists.
(7) Participate in the management of the ward and visiting and accompanying staff, turn out the lights on time, inquire about any suspicious person, and report to the superior in time in case of any important or abnormal situation.
(8) Increase the number of staff on duty on holidays, and the number of staff on duty during the day shall not be less than three, and the staff on duty shall maintain effective contact with the ward.
2. Shift handover:
(1) Ward collective shift handover once a day in the morning.
(2) Before handing over the shift, the nurse on duty should complete a good variety of nursing records and check the completion of various tasks to prevent errors or omissions.
(3) The order of shift handover is in the following order: nurse shift report, nursing record sheet, special circumstances and related notes, bedside handover, handover with the treatment shift and the main shift.
(4) The bedside handover is: critical, newly admitted, same-day surgery, being infused and primary care patients; the main handover of the patient's condition, treatment, nursing care, skin, fluid intake, the implementation of medical advice and the general situation of new patients.
3. Shift taking:
(1) Prepare for shift taking: neatly dressed, dignified and full of spirit.
(2) Participate in the shift, concentrate, listen carefully to the situation handed over by the shift personnel.
(3) The bedside shift handover, the content of the handover in doubt should take the initiative to put forward, in order to clarify the situation.
(4) Checking and counting poison and anesthetic drugs and related items and equipment in person, registering and signing. Medical equipment and drugs do not match the number of not handing over
7. Resuscitation items do not match not handing over
8. Medical orders are not checked not handing over
9. Critically ill patients' beds are not dry not handing over
10. Treatment rooms and offices are not tidy not handing over
