Application materials for change of medical institutions

Note: Application materials should be printed on A4 paper (except drawings), stamped page by page, and bound in order; if the submitted materials are copies, they should be copied with "copy of the original" and stamped with the official seal of the unit. The contents of the declaration should be true, complete and clear, and should not be altered. If the applicant does not come in person, the clerk should provide a power of attorney for the applicant.

1. Application for change of registration of medical institutions;

2. Application of hospital (red head document);

3. The original and copy of the original and copy of the License to Practice in Medical Institutions;

4. Other information required to be submitted;

(1) Change of name:

1) Relevant supporting documents for the change of name (documents of the superior Competent authorities documents, the organization's shareholders meeting minutes resolution, etc.);

(6) Change of registered capital

Recent proof of creditworthiness of the medical institution, i.e., a certificate of creditworthiness issued by a bank (original) or financial certificates issued by higher authorities.
