Medical institutions in the event of one of the following radiological events, should be promptly investigated and dealt with, truthfully recorded, and in accordance with the relevant provisions of the timely report to the administrative department of health and the relevant departments: (a) diagnostic radiopharmaceuticals, the actual dosage deviated from the prescribed dose of more than 50%; (b) radiation therapy, the actual dose of irradiation deviated from the prescribed dose of 25% or more; (3 (C) personnel mis-illumination or misuse of radioactive drugs; (D) radioisotopes lost, stolen and contaminated; (E) equipment failure or human error caused by other radiological events.
